• Responsible for assessing clinical trial protocols for feasibility, co-ordination of acceptance, documentary controls, drug storage and accountability, dispensary routines and financial management.
• Liaising with Grampian Clinical Research teams to develop and review protocols for locally sponsored studies.
• Responsible for creating and approving clinical trial documentation, prescription forms, worksheets and standard operating procedures ensuring compliance with Clinical Trials Regulations.
• Responsible for setting up and close out of clinical trials in Grampian ( including but not limited to cancer, cardiology, diabetic, GI trials), collaborating with chief investigators, research nursing teams, clinical research organisations, sponsors and R&D teams through trial lifetime.
• Supervising and supporting post registration pharmacists in their education, training and development.

• Responsible for managing clinical caseload for newly diagnosed hypertensive patients.
• Responsible for managing pharmacotherapy related tasks - acute and repeat dispensing, polypharmacy and medication reviews, medicines information related pharmacy queries, care home reviews, actioning hospital correspondence, reviewing high risk patients (i.e. patients prescribed DMARDs and DOACs), resolving high risk medication issues and liaising with patients to ensure therapeutic goals are being achieved and medication is optimised.
• Developing SOPs in collaboration with lead pharmacist to ensure clear and consistent medicine related procedures across the practice.
• Working closely with the wider MDT to ensure prescribing and clinical efficiencies are prioritised.
• Developing practice and local population specific pathways for identifying upstream early intervention of CVD and seeking modems for addressing this in line with Scottish government CVD DES initiative.

I worked across 2 fairly rural GP practices - Skerryvore Medical Practice and Heilendi Medical Practice. In addition to pharmacotherapy related tasks, I was responsible for :

• Actioning outpatient and hospital letters.
• Undertaking telephone medication review clinics for CVD, COPD and Asthma patients.
• Working closely with social care and district nursing teams to identify pharmaceutical care needs of rural patients.
• Developing novel ways of delivering pharmaceutical services to rural patients.
• Member, Hypertension workstream - Scottish Cardiac Audit Program aimed at improving patient care and outcomes across Scotland.

• Responsible for pharmacotherapy related tasks- acute and repeat prescriptions, medicines reconciliation, instalment prescribing, serial requests, and dealing with all pharmaceutical queries.
• Reviewing medication to ensure effective person centred care is achieved.
• Undertaking care home polypharmacy reviews with the wider multidisciplinary team.

• Responsible for providing clinical and on call services across a range of clinical specialities ( e.g. aseptic services, orthopaedics, geriatrics, oncology, medical and surgical wards) as part of a rotational team of pharmacists.
• Responsible for overseeing preparation, production and release of aseptically prepared cytotoxic medication and total parenteral nutrition (TPN)for oncology/haematology/paediatric and surgical patients.
• Provided clinical ward rotational and dispensary service to service users.
• Enhanced patient care by providing expert advice on prescription medications.

• Responsible for providing minor ailment, chronic medication, acute medication, smoking cessation and substance misuse service to service users.
• Clinical verification, dispensation and checking of prescriptions, ensuring medication is safe, effective, appropriate and person centred.
• Providing health promotion advice.
• Liaising with GPs and other HCPs to ensure seamless transfer of patient information and pharmaceutical needs as appropriate.
• Conducted thorough medicine reviews for optimal patient care.

• Provided clinical ward cover to Stroke, Medical and Surgical wards
• Clinical verification and check of prescriptions, ensuring the safe, effective and appropriate use of medication.
• Retrieving and recording patient medication histories and undertaking medication reconciliations.
• Assessing patients' own medication for reuse.
• Ensuring therapeutic monitoring of high risk drug types/regimens.
• Communicating with and advising other medical and allied health professionals on pharmaceutical issues in relation to patient care.
• Counselling patients on appropriate drug use following discharge.
• Receiving and responding to drug related enquiries as part of the medicines information team.
• Providing dispensing service to inpatient and outpatient departments as part of patient services.

• I worked in various departments under different clinical capacities in a structured programme. These included the Mental Health department covering both inpatient and outpatient prescriptions, Aseptic Department for the manufacturing of all aseptically prepared formulations, Medicines Information Department for the dissemination of evidence-based medicine, Dispensary services for the dispensing of all types of prescriptions, Clinical ward services with the responsibility of covering respiratory and cardiovascular wards and the Stores/production services.
• Achievement: Conducted a clinical audit to investigate antibiotic prescribing patterns and adherence to guidelines on ED ward. Results highlighted lack of prescribing uniformity and non-adherence to formulary guidelines. Undertook education and training sessions for junior doctors and re-audited patters. Re-audit findings demonstrated 75% improvement in standards.

• I provided a clinical service in conjunction with the directorate pharmacist. Responsibilities included: - Retrieving and interpreting correctly patient medical histories upon admission to the hospital. - Assessing their drugs for reuse and - Highlighting significant drug events to pharmacist ensuring appropriate outcomes.
• Achievement: Initiated and effectively executed an out of hours discharge service on 2 surgical wards in response to time and pharmaceutical stock delays hampering the discharge process.
• Collaborated with the surgical directorate pharmacist and completed extensive audit into the cost implications of medication transfer errors within the hospital. A key outcome was identifying reasons for transfer errors, and proposing practical recommendations to ensure seamless medication transfer
