- To ensure there is a clear rationale for referral and proposed care plan documented by the referring Clinician

- To support and supervise the Band 3 staff delivering intervention to patients open to the ICM team and countersign their notes on system One
- To carry out urgent assessments and ensure that care plans, risk assessments are relevant and up to date for all patients on the ICM caseload
- To liaise and work alongside the wider MDT and other services/ organisations including the Community care home team, Adult Social Care, and voluntary sector
