- My duties include redirecting OPMH referrals that require a crisis response and assessment within 4 hours or MHAA assessment to the Crisis Team
- To ensure there is a clear rationale for referral and proposed care plan documented by the referring Clinician
- To plan, implement and evaluate the care of patients open to the ICM team
- To support and supervise the Band 3 staff delivering intervention to patients open to the ICM team and countersign their notes on system One
- To carry out urgent assessments and ensure that care plans, risk assessments are relevant and up to date for all patients on the ICM caseload
- To liaise and work alongside the wider MDT and other services/ organisations including the Community care home team, Adult Social Care, and voluntary sector
- To participate weekly in discussion with the OPMH Psychiatrist and arrange medical reviews where appropriate
- To ensure that when a patient is ready for discharge to the CMHT, there is a discharge summary ready on S1 for the allocated CPN to access, if appropriate a verbal handover can be facilitated
