Throughout my three years of clinical placements within Midland partnership foundation trust, I have gained work experience in acute inpatient settings, forensic inpatient settings and community mental health services.

Throughout my experience in community mental health services, I effectively managed my own case load contributing positively to patient outcomes within early intervention services.

I have Developed a broad range of clinical skills essential for effective patient care.

• Risk assessment and risk management.
• Coordinated discharge planning process for smooth transition of patients into the community.
• Crisis intervention and de-escalation: Providing evidence based therapeutic interventions
• Observation and monitoring: Carrying out different levels of patient observations (general, intermittent, constant) to ensure patient safety and wellbeing.
• Organising and arranging the daily planner for the ward, ensuring smooth running of the ward.
• Multi disciplinary collaboration: Working alongside psychiatrists, psychologists, social workers, education and wider professionals to coordinate care.
• Physical Health monitoring: Supporting physical health checks such as General observations, blood pressure monitoring, clozapine monitoring and health promotion.
• Clinical skills such as safe medication administration. Safely administering medication in line with NICE Guidelines.
• Competent in the Administration of intramuscular medication in line with the NICE guidelines. Competent in performing venepuncture to obtain blood samples for diagnostic testing while maintaining patient safety and comfort.
• Therapeutic communication: utilising active listening, empathy and therapeutic engagement to build professional relationships with patients.
• Conducting physical health assessments for patients currently taking anti psychotic medication.
• Safeguarding and advocacy: Identifying safeguarding concerns and advocating for patients in line with NHS safeguarding procedures.
• Developing recovery focused patient centred care plans and reviewing these with patients and the multidisciplinary team.
• Conducting mental health assessments: Conducting holistic assessments of patients mental health needs including patients physical, emotional and social needs.

I have worked with Nurturing Haven since 2021 as a senior support worker. Throughout my role as a senior support worker I have provided high quality care for children and young people who are experiencing complex mental health conditions.
