As a Relevant Person’s Representative for vulnerable adults and specifically people with dementia, my responsibilities was, keeping contact with the service users, representing and supporting them in all matters relating to the deprivation of liberty safeguards (DoLS) for people who are lacking capacity. I was also made sure that they were having group works activities , self-work, visits, community activities, and meets with other citizens or family members. I had to applied positive service user approach to increase satisfaction levels and to built rapport with them.

• Allocated with cases with I had to assess service users, write reports, case recordings and referrals.
• Supported and advised service users regarding various issues and signposted additional services.
• Empowered individuals to lead normal, healthy lives by delivering professional and emotional care.
• Increased youth participation in pro-social activities through collaboration with therapist assistants and community resources.

Work within asylum seekers, complete benefit forms

and referrals to support service users and manage

caseloads under the guidance of my practice

supervisor. Directly work with individuals, carers and

families.

• Provide humanitarian aid, emotional support, coupons, etc.
• Assisted service users to obtain and access community resources.
• Offered practical daily living advice, including financial planning, housing support and legal guidance.
• Adapted support where possible to increase service accessibility.
• Assessed clients and documented information for records and analysis.
