
I am a patient, calm and person‑centred social work student with experience supporting adults, families and individuals with a range of needs. I am confident engaging in sensitive conversations and gathering clear information to understand what matters most to the person or family. Through my placement in Adult Social Care and my background in support work I have developed strong assessment skills, the ability to work with limited information and experience creating support plans that focus on strengths, independence and wellbeing.
I build positive relationships quickly, communicate in a respectful and clear way and adapt easily to different situations and levels of need. I am reliable, honest and organised and I approach every case with empathy, professionalism and a genuine commitment to supporting people in the best and most meaningful way I can. I enjoy working with both adults and children and feel confident applying my skills across different areas of social work practice.
During my placement in the Adult Central Neighbourhood Team I worked with a wide range of adults including older people after hospital discharge and adults referred with no information provided. I carried out full Care Act assessments from the start gathering all the information myself through person‑centred conversations with the individual their family and other professionals.
I arranged care packages, created support plans and reviewed short‑term reablement support to see if longer‑term care was needed, using the Care Act wellbeing principle (Section 1) and the eligibility criteria (Section 13). I also involved unpaid carers and made sure their voice and situation were included in the assessment.
I had the chance to nominate an adult for an Extra Care scheme, bringing together key information and presenting it clearly for panel. Throughout placement I became more confident with case notes, home visits, and talking about sensitive issues in a calm and respectful way.

At Nurseplus I supported adults with a range of needs, including learning disabilities physical disabilities and complex health conditions. I worked in different settings, which helped me become confident adapting to new environments and building positive relationships quickly.
My role involved helping people with daily living tasks, personal care, communication, and encouraging independence wherever possible. I always worked in a very person‑centred way, making sure people felt comfortable, respected and listened to. I got used to handling sensitive situations calmly and professionally, and keeping accurate notes when needed.
This job helped me build strong skills in patience, communication, empathy, and supporting people with dignity.
