• To undertake patient facing and patient supporting roles to ensure effective medicines use.
• Carrying out medicines optimisation tasks including effective medicine administration, supporting medication reviews and medicines reconciliation.
• Support Clinical Pharmacist in Structure Medication Reviews (SMR)
• Provide expertise to address both public health and social care needs of patients, including lifestyle advice & service information.
• Manage shared care protocols and liaise with Clinical Pharmacists.
• Assist delivery of medicines optimisation and management incentive schemes and patient safety audits.
• Support PCN multi-disciplinary team to ensure efficient medicines optimisation processes are being followed.
• Develop relationships with other pharmacy professionals and members of multi-disciplinary team.
• Support implementation of national prescribing policies and guidance within GP practices, and other primary care settings.
• Support PCN in reviewing and developing relevant policies for CQC requirements.

• To reconcile medicines from clinical letters and discharge summaries from secondary care settings.
• Work with practice to action MHRA drug alerts.
• Review medicine audits and protocols.
• Support pharmacists to undertake structured medication reviews.
• To reconcile medicines from appropriate up to date sources.
• To liaise with community pharmacy to ensure safe and timely supply of medication.
• Support implementation of national prescribing policies and guidance within GP practices
• Work to improving Quality and Outcomes Framework (QOF) within each practice.
• Process repeat prescription requests in line with surgery protocols.
• Review and adjust repeat prescribing protocols to improve patient safety and reduce medicine wastage.
• To review out of stock medications and suggest appropriate alternatives to prescribe.
• Develop relationships with other pharmacy technicians, pharmacists including those in primary care, secondary care, community pharmacy, mental health, and social care teams.

• Supporting preventative medicines projects, Process medication requests, Processing prescriptions in line with agreed clinical protocols, Support medication reviews and medication reconciliation, Ensuring effective medication use through shared decision-making conversation with patients, Developing & maintaining medicine related comms to all practice staff, Point of call for customer & patient queries, Liaise with clinicians & staff to resolve prescribing queries, Update & maintain accurate patient medication records, Support PCN team with medicine optimization

• Key holder responsibility for store, Prepare, label & dispense medication, Stock take & procurement of medicine & products, Answering customer queries around medication, Maintain electronic database in relation to customers & ordering, One to one support and advice for customers in store, Training new team members, ensuring standards are maintained, Providing wide range of services including supervised consumption, needle exchange, STI testing, blood pressure checks & stop smoking support

• Supervisory duties in team leader & manager absence, Prepare, label & dispense medication, Providing high level of customer service in store, Answering customer queries around medication, Maintain electronic database in relation to customers & ordering, One to one support and advice for customers in store

• Dispensed medication accurately against customer prescriptions.
• Answered incoming phone calls and addressed questions from customers and healthcare providers.
• Maintained patient confidentiality while handling sensitive medical information.
• Maintained proper drug storage procedures, registries and records for controlled drugs.

• Obtained qualifications as Medicines Counter Assistant, Dispenser and Pharmacy Technician, All Pharmacy Technician duties as above

• Customer facing, Moulding & designing cake decorations for major clients such as Morrisons, ASDA & Tesco, Boxing & packaging ready for deliveries, Stock replenishment, Stock ordering & forecasting product demand, Controlling wastage to maximise profits
