This Veteran Affairs National-level specialty pharmacogenetics program requires CPhT to use their independent technical judgment to provide comprehensive patient-centered care. The role demands a wide range of skills, including a thorough knowledge of pharmacogenomic practice, clinical workflow, organizational knowledge, problem-solving, communication, and coordination encompassing clinical, educational/training, and administrative skills to uphold complex pharmacy practices, regulatory requirements, and quality standards.

• CPhT learning to conduct patient interviews including medication history, pharmacogenomic personal and family histories, collection and input of pharmacogenomics results, utilization of PGx results, medication adherence and barriers therein, need for medication refills, and investigating causes of therapeutic irregularities and nonadherence.
• Absorbing communication style related to changes in PGx informed medications and/or dose with patients and accurately document all communication in the patient's electronic record. This includes reporting of adverse drug reactions and incident reporting.
• Ascertaining skills to complete consenting for pharmacogenomic testing, review patient lab results, and triage per protocol and documentation of pertinent patient information for pharmacist review as appropriate.
• Will be required to participate in medication utilization evaluations, quality improvement projects, dashboard review and utilization, data analytics, and quality reporting as assigned through by NTPGx/NPP.
• Understating how to provide supplemental reinforcement education for patients concerning operational issues or need for therapy compliance. Creates or assists in creating Patient Education Materials; works with appropriate facility committee to gain approval for patient use.
• Required to triage phone calls, voicemails, emails, and other messages from internal and external customers. Fax, email, or mail correspondence to appropriate patients and staff. Communicate with outside entities to gather information for NTPGx/NPP.
• Training to assess and distribute the patient care workload to staff assigned to clinical area, provide feedback, and participate in project improvement.
• Will contribute to QA/QI projects, medication utilization evaluations, safety initiatives, dashboard utilization, aggregation, and review, other data analytics, and cost savings initiatives as assigned.

To provide comprehensive support for oral and intravenous chemotherapy and support treatments, CPhT collaborated with clinical oncology pharmacy specialists, practitioners, and oncology staff across multiple services to support the complexity of medication therapies utilizing clinical knowledge and experience to evaluate and analyze the effectiveness, compliance, and tolerance of a variety of oncology/hematology regimens.

• Conduct patient interviews, including medication history, adherence, effectiveness, compliance, toxicity, and assess appropriate follow up and laboratory monitoring based on established oral chemotherapy order set and national dispensing reference guide.
• Central contributor of a research project published by pharmacy residency program developed to analyze, improve, and establish a pharmacy ran comprehensive oral chemotherapy monitoring program.
• Employ and uphold department/hospital standards to maintain The Joint Commission accreditation at our facility to ensure safe and quality clinical care to all patients.
• Key expert in the implementation of a CPhT driven oral hematology chemotherapy monitoring procedures to ensure appropriate clinical care standards and patient safety. This included creation of work processes, analysis of workload, template development, collection of feedback, utilization of dashboards to create statistical reports, and the creation of future training documents, competencies, and standards with pharmacy leadership.
• Attend and contribute to quarterly VISN 17 Hematology Oncology Community of Practice Meetings providing decision support to VISN P&T regarding hematology/oncology topics.
• Participate in QA/QI processes and projects such as resident rotation improvement projects for clinic establishment and operation.
• Accurately assess and document patient's condition and treatment compliance and comprehension via telephone and face to face encounters and accurately document all communication in the patient's electronic record. This includes reporting of adverse drug reactions and incident reporting requiring independent judgement making to provide to appropriate handoff when necessary.
• Provide patient education, appropriate treatment guidance, and materials related to treatments.
• Association of VA Oncology Hematology member which encompasses a multi-professional collaboration and networking, increased research capabilities, and educational opportunities to include attendance in 3 day national VA Oncology/Hematology educational symposium in October 2023.
• Attendance in monthly 2023-2024 Pharmacy Grand Rounds providing continued education and attainment of pharmacy knowledge related to medication therapies, treatments, and patient care.
• Created training manual to orient and train new clinical oncology technician to clinical role and established technician scope of practice related to responsibilities, expectations, daily tasks, charting requirements, and workflow processes.
• Worked with centralized pharmacy services, pharmacy leadership, and procurement to contribute to the development of interdepartmental SOP to improve fiscal accountability, documentation, and resolve charting and patient safety issues related to medications monitored by Risk Evaluation Mitigation Strategies Programs (REMS).

Clinical pharmacy technician position supporting clinical pharmacist practitioners servicing disease management for chronic conditions such as diabetes, hypertension, hyperlipidemia, and anticoagulation.

• Oversee clinic coordination efforts to improve patient outcomes by accurately interpreting medication orders, clinic consults, and medication needs in a timely manner.
• Assist in establishment and implementation of procedures that ensure the delivery of quality goal oriented optimum patient care maintaining and ensuring compliance with The Joint Commission standards.
• Collect data, import data, statistical analysis, and creating graphical presentations; in addition the knowledge of correct formatting and compilation of flow charts.
