This Veteran Affairs National-level specialty pharmacogenetics program requires CPhT to use their independent technical judgment to provide comprehensive patient-centered care. The role demands a wide range of skills, including a thorough knowledge of pharmacogenomic practice, clinical workflow, organizational knowledge, problem-solving, communication, and coordination encompassing clinical, educational/training, and administrative skills to uphold complex pharmacy practices, regulatory requirements, and quality standards.

  • CPhT learning to conduct patient interviews including medication history, pharmacogenomic personal and family histories, collection and input of pharmacogenomics results, utilization of PGx results, medication adherence and barriers therein, need for medication refills, and investigating causes of therapeutic irregularities and nonadherence.
  • Absorbing communication style related to changes in PGx informed medications and/or dose with patients and accurately document all communication in the patient's electronic record. This includes reporting of adverse drug reactions and incident reporting.
  • Ascertaining skills to complete consenting for pharmacogenomic testing, review patient lab results, and triage per protocol and documentation of pertinent patient information for pharmacist review as appropriate.
  • Will be required to participate in medication utilization evaluations, quality improvement projects, dashboard review and utilization, data analytics, and quality reporting as assigned through by NTPGx/NPP.
  • Understating how to provide supplemental reinforcement education for patients concerning operational issues or need for therapy compliance. Creates or assists in creating Patient Education Materials; works with appropriate facility committee to gain approval for patient use.

Oncology Clinical Pharmacy Technician

  • Central contributor of a research project published by pharmacy residency program developed to analyze, improve, and establish a pharmacy ran comprehensive oral chemotherapy monitoring program.
  • Employ and uphold department/hospital standards to maintain The Joint Commission accreditation at our facility to ensure safe and quality clinical care to all patients.
  • Key expert in the implementation of a CPhT driven oral hematology chemotherapy monitoring procedures to ensure appropriate clinical care standards and patient safety.
