Final Placement – Older People’s Mental Health Team Southern

  • In my current placement with the older people’s mental health team, I engage in a diverse range of responsibilities aimed at supporting individuals and families.
  • This includes conducting risk assessments, developing risk enablement plans, and completing various assessments such as Care Act assessments, mental capacity assessments, and s117 reviews.
  • I prioritise maintaining confidentiality and adhering to data protection protocols while accurately recording case notes on Mosaic.
  • Building rapport with customers and effectively communicating, both verbally and non-verbally, is crucial in navigating challenging situations within a potentially stressful role.
  • I manage caseloads, work collaboratively as part of a team, and communicate with customers and their families in a person-centred and strengths-based manner.
  • Additionally, I utilise community resources, signposting, and referrals to internal and external services to meet the needs of the individuals I support.

1st Placement – Youth Justice Worker

  • My first placement was within the youth offending team as a youth justice worker.
  • In this role I designed and implemented interventions for young people, collaborating closely with schools, parents, medical professionals, and therapists to ensure holistic support.
  • I maintained accurate case notes on Mosaic and CorePlus, completing risk assessments and safety plans in line with relevant legislation such as the Children Act 2004.
  • By building rapport with young people and their families, I facilitated access to community activities and encouraged their engagement in education and other pursuits/interests.
  • Utilising skills such as kindness, patience, and motivational interviewing, I fostered a non-judgemental and supportive environment, adapting interventions to meet individual needs.
  • I referred young people to external agencies for specialised support in SEN, housing, and substance misuse services, while prioritising data protection and risk management.
