• I assess the mental, emotional, and physical needs of Residents to create individualised care plans.
• Participate in Hurdle and Ward rounds
• I provide compassionate, person-centred care that supports residents’ mental health and overall well-being.
• I administer prescribed medication safely and monitor its effectiveness and any side effects.
• I give Nursing care and support residents with conditions such as dementia, eating disorder ,bipolar disorder,schizophrenia ,depression, anxiety, and other mental health challenges.
• I build trusting relationships with patients , their families, and other members of the care team.
• I contribute to multi-disciplinary team meetings and collaborate with GPs, psychiatrists, and social workers.
• I offer emotional support and psychological interventions tailored to each resident’s needs.
• I maintain accurate and up-to-date records in line with NMC standards and organistaion'sCare’s policies.
• I ensure safeguarding procedures are followed and act promptly if any concerns arise.
• I promote independence, dignity, and quality of life for every resident under my care.
• I provide guidance, mentorship, and support to care assistants and junior staff.
• I participate in training sessions and stay updated with best practices in mental health nursing.
• I contribute to creating a calm, safe, and therapeutic environment within the care home.
• I respond to mental health crises or behavioural challenges with professionalism and empathy.
• I uphold confidentiality, equality, and respect in every interaction I have.
• Also assist with ADL's of the patients

ROLE PURPOSE / SUMMARY OF RESPONSIBILITIES

I worked in the elderly unit of caring cross specialist hospital ;my duties and responsibilities include but not limited to;

Conduct comprehensive assessments of elderly patients.

Develop individualized care plans based on patient assessments.

Administer medications as prescribed and monitor their effectiveness.

Monitor vital signs, cognitive function, mobility, and nutritional status.

Provide assistance with activities of daily living (ADLs) such as bathing and dressing.

Manage and treat wounds, pressure ulcers, and other skin conditions.

Implement fall prevention strategies and interventions.

Provide palliative care and support to terminally ill or chronically ill patients.

Communicate effectively with patients, families, and the healthcare team.

Advocate for patients' rights, preferences, and needs.

Document and maintain accurate patienGRADEt records.

Educate patients and families about disease management and healthy lifestyle practices.

Provide emotional support and address psychological needs.

Assist with end-of-life discussions and advance care planning.

Collaborate with physicians, therapists, and social workers to ensure holistic care.

Evaluate and monitor the effects of medications and treatments.

Implement infection control measures to prevent the spread of diseases.

Coordinate and facilitate interdisciplinary team meetings.

Respond to emergencies and provide immediate medical assistance.

Ensure a safe environment for elderly patients.

Provide patient and family education on medication management.

Assess and address pain management needs.
