Through this post at a high-volume primary PCI centre, I have developed broad experience in general and interventional cardiology through cardiology on-calls, CCU ward rounds, moderating regular PCI multidisciplinary meetings, and frequent participation in Joint Cardiology–Cardiothoracic (JCC) case discussions. The role has also provided extensive exposure to invasive cardiac procedures.

  • Coronary angiography/PCI: Overall experience across more than 400 cases
  • Diagnostic coronary angiography: Performed more than 250 cases with 6 DOPS signed by 6 different consultants, achieving Level 3 competency Most cases were performed via a transradial approach, both conventional and distal, with a small number via a femoral approach Additional experience includes a small number of graft studies, LV grams, and aortograms
  • First-operator PCI experience: Able to perform simple PCI under direct supervision Performed more than 60 directly observed PCIs
  • Second-operator PCI exposure: Additional second-operator exposure across a range of more complex cases, including graft PCI, LMS bifurcation PCI, CTO intervention, rotational atherectomy, intravascular lithotripsy, and unstable STEMI cases requiring haemodynamic support, as well as complex high-risk indicated PCI requiring mechanical circulatory support or right heart catheterisation for haemodynamic monitoring
  • Temporary pacing wire insertion: Performed with limited supervision; experience in more than 10 emergency TPW cases (a mix of RIJ and femoral access) gaining gradual indepedence, with additional experience of RFV access and RV lead placement in more than 15 cases during EP studies
  • Pericardiocentesis: Performed with limited supervision; involved in 10 cases, including 7 as first operator (subcostal access) and 2nd operator for 2 cases via apical approach
  • Echocardiography: Competent in transthoracic echocardiographic image acquisition and interpretation; scans performed independently with senior review of final imaging and reports Performed more than 150 echocardiograms, passed BSE theory, and currently progressing through the deanery echo delivery tool for core competency
  • Pacing/devices/EP: Exposure to more than 100 cases in total, able to perform simple PPM insertion under direct supervision
  • Other procedural skills: Independent in chest drain insertion, abdominal paracentesis, lumbar puncture, direct current cardioversion, and ultrasound-guided arterial and central venous access
