Summary

As highly autonomous, experienced and motivated Physiotherapist, I am driven to support clients after injury and illness to achieve their maximum potential, through comprehensive assessment and treatment, in whichever environment the client feels most benefit. I am confident working alone or collaboratively with an MDT to achieve the best outcomes for all those involved (clients, family, external agencies). I am extremely organized, both with planning my own diary and caseload management in addition to my previous Clinical/Team Lead duties and supporting the team within the Auckland region and extending across New Zealand. I am able to quickly build strong professional relationships with my clients, other clinicians and external stakeholders through effective and clear communication both verbally and written.

I have been able to develop and progress both my skill set and roles/responsibility while working as a junior/senior Physiotherapist in the UK for 4 years and additionally when I worked in New Zealand over the past 8 years.

This current employment includes work and progression as a Physiotherapist, Senior Physiotherapist, Lead Physiotherapist, Clinical Services Advisor and now Clinical+Team Lead.

Within my Physiotherapist scope of the role:

  • Community based rehabilitation to clients within the Auckland and Waikato region ranging from 13-99+ years old. MDT working with Occupational Therapists, Speech and Language Therapists, Dietitians, Psychologists, Social Workers, Rehabilitation Coaches, Nurses, Medical Specialists, NeuroPsychologists, GPs, ACC Case Managers and Assisted Recovery.
  • ACC contracts = Training for Independence TI and TIAS, Concussion, Single Discipline Assessments, Pain Management, Functional Capacity Evaluations.
  • Additional private referrals (Stroke, Parkinson's) and DHB Falls and Balance Programs.
  • Client conditions assessment and rehabilitation = Orthopedics, Spinal Cord Injury (referrals from the Auckland Spinal Unit), Traumatic Brain Injury (referrals from Auckland Brain Injury Unit), Concussion (vestibular Ax and Rx), Elderly, Falls, Multi-Trauma, Amputee, Mental Health and ACC Sensitive Claims.
  • Comprehensive assessments of ACC injury related and non-ACC client based needs in the community and clinic environment.
  • Developing individualized, client centered treatment plans and SMART goals for return to independence or work including extensive use of clinical outcome measures.
  • Report writing as per ACC and contract specifications (TI, SDA, FCE etc).
  • Clear, effective communication with clients, family, MDT, ACC and other external agencies both verbally (face to face, telephone, Telehealth) and non-verbally (emails, texts, reports).
  • Excellent ability to react and adapt to changing workload requirements to meet service demands.
  • Very efficient time management and organizational skills - notes writing, report writing, billing, client appointments, responding to emails/texts/telephone calls.

Within my Clinical + Team Lead scope of the role:

  • Providing clinical support to a large and diverse team across the Auckland region (and for a 6-month period, additionally the Waikato and Taranaki regions also).
  • Independent running of the monthly TI Meeting across Auckland - providing updates on contract specifications and support for client based queries and success stories.
  • Clinical + Managerial Coaching on a 1:1 basis with team members
  • Triaging and allocation of all TI referrals across Auckland (for all disciplines)
  • Providing additional Key Worker and clinical support on TI contracts for those clinicians who are less confident and less familiar with processes.
  • Report/costing sheet checking as required for all MDT clinicians across Auckland. Additional support to Northland and other regions of New Zealand as needed.
  • Supporting HR and recruitment, including review of CV's, conducting interviews, on-boarding new staff and inductions to the company and service.
  • Monthly and bi-yearly audits for the TI Service.
  • Completing yearly performance development reviews as Team Lead.
  • Involved in and contributed to service and regional development alongside Service and Regional Leads in management meetings and company strategies.

Band 6 (Senior) Physiotherapist

  • Lead responsibility for therapy services to a residential 27 bedded rehabilitation unit.
  • Interdisciplinary and multidisciplinary team working – therapy assistants, technical instructors, Occupational Therapists, Social Workers, Nurses, GP’s, Speech and Language Therapists, and Dietitians.
  • Assessment and treatment of a wide and diverse range of clinical conditions e.g. Stroke, Parkinson’s, Multiple Sclerosis, Dementia, Falls, Post Surgical patients (e.g. Hartman’s procedures), COPD, Orthopedics including spinal fractures, MSK conditions, Amputee’s, Palliative patients and Rheumatology.
  • Triaging referrals independently and with the MDT.
  • Responsible for providing accurate and timely information regarding patient’s rehabilitation progress and ongoing discharge planning during daily handover’s with wider MDT.
  • Organization and planning skills on a daily and weekly basis with regards to patient’s and also professional development of myself and other team members.
  • Responsible for holistic patient assessment, goal planning and overall ongoing treatments of all patients in the unit with assistance from technical instructors and occupational therapists.
  • Planning and implementing individual and group based treatment
    sessions.
  • Completion of patient audits, report writing, referrals (e.g. community therapy, orthotics, and wheelchairs), clinical letters to GPs or consultants.
  • Provide in-service training sessions to assistants and technical instructors on a monthly basis in addition to clinical coaching/supervision and supervision of students on placement.
  • Provide leadership and delegation with therapy and non-therapy staff in the unit.
  • Communication with patient’s and family members during assessment, treatment and discharge planning - including in challenging situations and difficult decision making.
  • Completing independent and joint access, home and discharge home visits.

Band 5 (Junior) Physiotherapist
