• I was a community matron working along side five other matrons- the team was commissioned in 2014 as part of the Quest for Quality report in 2011. The team worked within a primary care setting looking after residents in care homes across the whole of Calderdale to prevent GP call outs and hospital admissions.

I left my position of a staff nurse in 2008 to work in the community, i provided assessments and care planning for residents living in residential and nursing homes.

The role was to support care homes and signpost to other agencies where needed and to support mental health wellbeing within our MDT.

During my time in this post i was pleased to be part of several audits POMH which looked at antipsychotics and Lithium treatment, i also did 12 month secondment as a practice placement educator and was involved in facilitation of creating student nurse placements.
