Case holder for a team of District nurses , Undertaking all district nurses responsibilities , attending home visits for new patients and more complex patients ,Nursing duties include the care of the diabetic patient , extensive wound care , palliative patients , IV medication , all other types of medication, reviewing patients as they progress through the District nurses service , Triaging all team referrals allocating patients to the correct member of the team , Allocating case load to the team scheduling daily weekly visits, attending MDT meetings with all members of the wider nursing n GP doctors and social workers , liaising back to the team any changes that the trust are implementing , mentoring new members of staff students and staff members that are progressing and undertaking new skills , Ensuring the smooth running of the team
