This role consists of me reviewing follow up patients to progress mobility/ transfers to preadmission baseline. This includes me issuing equipment initially and facilitating rehab of patients on the ward before medically fit for discharge, or to complete community physiotherapy referrals and group classes. Follow up sessions are also used to flag up any issues that may arise for discharge and liaising with the appropriate team.
