As a dedicated Heath Care Support worker. I have worked for the NHS for 29+ years with a proven record in patient centred care. working in the Hospital and in the community in patients homes supporting those with a palliative care Diagnosis, helping to support the families and loved ones through Difficulties in their lives. Repositioning patient 's 4 hourly to prevent pressure areas. maintaining dignity, making sure their every need is taken care of with compassion and Dignity,.

Having a Flexible learning approach for both Palliative and Rehabilitation, working with cancer patient's and other serious illnesses like Covid, to Fractures, stroke, Falls, plus minor illnesses. Promoting individuals maintain their own independence as much as possible but with support when required.

My current post is working in the mid Yorkshire NHS Trust, As a dedicated healthcare support worker. I have worked in continuous employment for the NHS for 29+Years with a proven record of patient centre care. Working in the Hospital and in the community in patient homes, for both Palliative and Rehab patients. Whilst Supporting Patients with Palliative diagnosis, I'm proficient in maintaining all personal care need, making sure there is a correct DNAR ( Do not attempt resuscitation) form if patient has agreed to this prior to going home

Repositioning every four hours to maintained skin integrity,

To prevent pressure areas, With use of different equipment required Glide sheet, Hoist, wheel chairs, Wheeled Commode, profiling beds. Most importantly communication being skilled in observations.

Making sure the information is passed on quickly. This is done by using a lap top computer on my rounds, after every visit to inform the G.p or District Nurses or the OOH ( out of hours team) that the patient requires a stat dose of medication for Pain relief, Secretions, Agitation or Sickness. this is so the patient gets the medication required within the 1 Hour time frame. I can have up to 22 patient visits and more with staff shortage's per day. depending on the how many visit a patient has.

Helping to support their Families through this difficult time. being sympathetic to their needs. Conducting the conversation with care and compassion, giving them Time to talk. Giving them information in other services that may help such as, Marie curie services, or Hospice care, and my team the Integrated care team which provides all of their everyday living requirements, helps them make Choices that they would like, to ensure their care and comfort needs, are met.

I work with a Multidisciplinary team for those with a Rehabilitation need. Following individual care plan's from Physiotherapists - Doctors, District nurses, among other Health care professional's. These patient may have complicated needs, and can't managed without support from outside the Family, things like Covid to complicated fractures- Falls- Stroke etc, to Minor illness's.

Encouraging individuals independence and self-belief.

Supported patients through personal care tasks such as toileting, showering Feeding, maintaining sensitivity to needs, ensuring comfort, dignity, and safety at all times. Though some have been challenging, this was met with a calm and sensitive nature to achieve an optimum outcome.

Keeping Strict confidentiality of patients' information on a need to know basis. Reporting accurate information regarding patients notes is timed and dated.

Monitoring individual's physical and emotional well being, promptly reporting any changes to nurse in charge.

Kept patient environments clean and tidy, improving hygiene standard.

Maintained up-to-date knowledge on latest care practices by attending regular meetings and training opportunities.

Making sure all equipment used, is cleaned prior to use, after use.

Maintaining all polices, procedure are to NHS Guidance.

All mandatory training is kept to 100%

  • I worked for sure start a home start volunteer, whilst still working full time for Mid York's NHS, with the local Council. assigned to a family who were having problems, Duties were keeping records of family interactions with each other, taking the family out.
  • Taking notice of any Child abuse, or abuse to the mother, if there was any visible bruising to Mother or children, Inform the service if Father had been around, father had a court order to stay away. This work was very rewarding, it was sometimes hard, when the father was in the home, being very aware he was quite volatile, making decisions to use verbal skills learned to keep the tone light when speaking.
  • The skills learned in previous roles at chorley hospital on Hazelwood ward, a ward for those with mental health needs and challenging behaviours.
  • Interacting with individuals on their level to communicate calmly, within a non judgemental way.
  • Encouraging the Father to leave for his own interest, and that of the Family. I left this Position due to looking after my grandson on my days off, to help my daughter and her husband with childcare.

I worked part time in day Hospital, with dementia patients who came to day unit for respite care, It was my role to manage several duties, that included, music groups, art groups with other colleagues, quiz groups, closed groups.

I assisted with meals and afternoon teas.

I would go and work part time on Hazel-wood ward where patients would be admitted for psychiatric treatment, sometimes they would be on section, 1. 2 .or .3 to maintain their safety. It was secure ward for those with several different mental health Disabilities. This work was very challenging at times but very rewarding too,

I took a course in self defence over three days to show me how to handle and restrain someone if required to do so for my safety or that of someone else according to trust policies.

  • I began working for the social services, as a break from being a stay at home mum for a few hours a week, to get me out and about and back into the community.
  • I was given the role as a home visitor. To check on the patient in the community. To find out what they needed. and write a report of my findings.
  • A lot were very lonely and didn't see anyone for weeks. this is were I became good at listening, and reading between the lines. knowing when they really required help.
  • I did this for about a year.
  • This gave me the Encouragement to apply to be a Health care support worker at Chorley Hospital, Because I enjoyed working with People, and helping make a difference to their life,
