My main duities here consist of

sourcing homes for new born to 21 yaer old adults

Creating good working relationships with external and internal providers

to ensure that the

Sourcing and placing young people in either foster care/ residential/ semi-independent placements based on their needs

· Negotiating costs with prospective providers to fail in line with council budgets

· Liasing with sw on regards to placincare package review g Yp

· Attending and arrange placement meetings/ strat meetings/ review meetings/ care package review meetings/ stability meetings

pre QA visits to homes that the council have not worked with before

ideally completed before the YP moves in

Full QA visits after the YP has moved in

· Uploading documents/ case notes

· Check ins - 24/72/ 1 week with Yp once in placement to see how they are doing

and this also allows for any issues to be dealt with ina toimely mannar to prevent potential break down of placement

· Sending IPA and waiting for them to signed

· Answering telephone making calls to providers social workers

· Maintaining duty box- This is one a month assigned to montior the duty inbox

and pick up any emergency homes that are needed for same day -

· Maintaining my own email tray and ensuring that it is organised

Working on own initiative and able to work effectively in a team when needed

New/ amend homes on mosaic - retainers / respite

For every yp placed HSR needs to be completed which deatils - where the yp was placed , previous placement details / costings /cla notifiactions /SPRF/Contract between provider and SS /welcome cards etc

Liasie with inhouse fostering when Homes are required for a foster carer

Liaise with finance in regards to payments for Providers / resipite/ retainers

  • Maintained detailed records of client interactions, interventions, and outcomes for reporting purposes.

whilst i worked in croydon council - i worked with two differnt teams -

I had a hand in introducing a new service into not only croydon ault services but also to the hospital discahge team that is reablement team

I was trained for two weeks prior to the launch of the new service and within the training weeks i shaowed reablenment officers on visits within the community after the patients had left hospital and had reablenet care packages in their homes - the visits were to ensure that they had the correct level of care and that their needs were met to help them reable back into the community

As a TRIAGE OFFICER my role was to triage referrals sent from hospitals , if ther was not enough information then the referral would be sent back to be corrected -

if the care packge was too much or not enough we would asses there needs - through lookingtalking to allocated sw , checking their medical history and frinding out whether they had family friends to help within the community and sometimes amend care package

· Updating case notes

· Restarting packages of care

· Ending packages of care

· Liaising with care providers to ensure package of care has started once sourced

· Sending confirmation emails to care providers / hospital discharge team and triage team with support plan attached

uploading documents liquid logic and writing acse notes

After reviewing referral we would send to brokerage team to source provider that could facilitate care within the home of that particular patient

This was a very varuied role and i lewarnt alot about suits , fabrtics , sizes. and cuts

Throughout my time at Moss bros I learnt the infamous Tailor me process - this involves sizing the customer using a block model cut either a slim fit. tailored fit or regular fit and which every cut fits the customer - i would then take the suit measure the lenth of the sleeves of the jacket the lenght of the body of the jacket and the width of the jacket - trousers it would be the waist and leg length then i would input these measurements into the system and then continue to ask the customer a list of questions whilst handing buttons ,fabric for suit/ and inside of jacket and whether they want the suit peronalised - once i have gathered all the information i would ak for their personal deatils and let them know when they can pick up their suit -

i also sold suits from the rracks and on ocassion would be required to pin a jacket or trousers in acse they required a alteration

I also worke on the tills

in the stockroom to take deliveries

and re organise the stockroom when new stock arrived

I also was asked to go to other stores and re organise their stockrooms as my work

was benefical to the company and i had great feedback from area mangers

I also would train new starters

whilst i worked here i also studied and completed a diploma level 4 in garment technology which i was very proud of and this helped me immensleywithin my role

i also did some work exoperience for my course with the garment technology department of Moss bros
