Organise my clinic on a day to day basis

See patients for , Blood Tests, Injections, Assessments, Dressings, Observations, Weight Management Calls.

Restock clinic room at the end of the day.

Liase with Gp, rest of nursing team if required.

Refer patients onwards for treatments such as physio , Gp appointments. End of Life care

Computer work, Documenting the appointment.

Day to Day care for End Of life patients.

Personal Care

Comfort and reassure patients and their families

Reporting to senior nurses after seeing patients.

Helping with medication.

Answering calls for emergency visits for End Of Life Care and blocked Catheters.

Open referrals

Speak with Case Coordinators, Nurses, Social Workers etc

Add patients to planners for Nurses to be allocated.

Using System 1

Support 18 + Adults coming out of care to live in their own home.

Shift work, Working Day and Nights.

Supporting with Medication

Helping with Housework etc

Attend appointments with clients.

Suuport with making Gp appointments.

Out Of Hours Nursing

Emergency Calls for Catheter Care and End Of Life patients

Urgent Dressings

Speaking to the Gp's, Ambulance and police when needed.

Documenting visit with patient on System 1

Assist with setting up Syringe Drivers

Liasing with familes of the bereaved

Supporting patients at home with :

Dressings, Injections , Observations, Assessments, Catheter Care, Giving Insulin, Wound Mapping,

Refer for other care if needed.

Consulting with Gp's Nurses, Physio etc

Using System 1 and Rio
