  • This includes creating and reviewing safety plans which are person cantered to meet the needs of the individual, ensuring the young person is safe in their home environment.

  • Ongoing Support
  • To be able to signpost child/young person to services which will offer ongoing support after initial crisis period
  • Engage family/carers in formulating a safe discharge plan and coping strategies
  • Involving family/carers in care planning and educating them about young person’s mental health difficulties; providing advice on how they can support them when in a crisis situation.
  • MDT:
  • Working as part of a multi-disciplinary team, working with and liaising with other professionals to ensure safe and co-ordinated care for children in a mental health crisis
  • Attend multi-agency meetings to provide holistic care which meets child/young person’s physical, emotional and social needs.
  • Mental Health Act and Mental Capacity Act
  • Sec136 – managing patients brought in by the police on Sec 136
  • Co-ordinating and arranging the Mental Health Act assessments
  • Co-ordinating with bed managers if patient needs hospital admission.
