-Complete an assessment of needs and build a person-centred support plan to ascertain priority needs and sign posting required
-Promote independence through education and support to gain life skills such as budgeting and paying bills, IT skills and engaging in the community
-Sign post/refer and support with engagement with Drug and Alcohol groups
-Work collaboratively with health professionals such as LA, Adult Social Care, Charity Organisations, GP's, Social Prescribers and MH professionals.
-Ensure all concerns around Safeguarding are reported effectively as per policy. Attend all meetings (CIN, TAF, Enquiry Planning meetings, MARAC referrals) as appropriate.
-Manage a caseload in appropriate time ensuring all keyworks and related actions and paperwork are recorded in a timely manner
-Provide residential and educational support for YP aged 16-24 diagnosed with ASD, ADHD, Anxiety Disorders, PDA and other co-morbidities.
-Deliver Person-Centred care as per the wishes of each individual, their Care Plan and future progression
-Complete weekly 1:1 sessions with key students and ensure these are recorded and any actions followed through.
-Create Risk Assessments for both onsite and offsite activities as well as ensuring Risk Assessment Matrix is updated for each YP as necessary.
-Accompany YP on trips off-site and ensure their safety as well as the safety of the general public
-Create and lead training sessions for staff
-Support YP to learn and engage with education as well as develop life skills and gain independence.
-Provide support for adults with Downs Syndrome and Learning Difficulties to effectively run a Coffee shop and Community that is open to the public
-Support includes Money Management (using a cash register, handling cash and giving customer change) Building social skills and customer service such as taking food orders, delivering food and drink and learning basic social etiquette.
