My Key responsibilities include

• Team leading and supporting staff to maintain high standards of care throughout each shift.
• Administration of medication safely and accurately in line with regulatory guidelines. As well as Overseeing Advanced Nurse Practitioner (ANP) ward rounds to support integrated clinical decision-making.
• Reviewing and updating care plans and assessments to reflect residents' evolving needs.
• Delivering personal care with dignity and respect, ensuring residents' comfort and hygiene.
• Monitoring and recording fluid intake, bowel charts, and other clinical observations to identify any areas of concern promptly.
• Liaising with multidisciplinary teams (MDTs) including GPs, ANPs, therapists, and external health services to coordinate holistic care.
• Making appropriate continence referrals and ensuring timely follow-ups.
• Scheduling medical appointments and coordinating transport arrangements to ensure continuity of care.
• Maintaining open, respectful communication with residents and their Next of Kin, fostering trust and involvement in care planning.

My Key Responsibilities included:

• Collaborating closely with multidisciplinary teams to provide personal care and ensure comfort for post-surgical patients. It required adaptability in a high-turnover environment. This role required attention to detail, and the ability to respond promptly to changing clinical needs while maintaining a compassionate presence.

I was accountable for:

• Monetary management of client finances.
• Coordinating and carrying out shopping on behalf of residents with different level of social needs and sociecomic accesses, ensuring their personal preferences and needs were met.
• Scheduling and attending appointments to support residents in accessing necessary services.
• Providing medication control, guidance, and support in line with best practices and individual care plans.
• Offering emotional support and informal counselling to promote residents' mental and emotional well-being.

Throughout this role, I developed strong initiative and decision-making skills, often taking the lead in situations that required quick thinking or sensitive handling. My ability to manage responsibilities effectively and build trust has made me a dependable and valued presence for both residents and colleagues.

• During a year-long placement, I shadowed a variety of professionals—including family support workers, mental health support staff, and adult care assistants; which enhanced my ability to provide tailored support. This experience developed my adaptability, empathy, communication skills, and teamwork in diverse care settings.
