I am a UK based junior doctor currently working at a PGY-2 level in the NHS in UK. I have a passion for clinical medicine and I have over 2.5 years of resident medical officer (RMO) experience in general medicine wards, Coronary Care Unit (CCU) and acute psychiatric ward in busy, tertiary care hospitals. My clinical experience has enabled me to develop the necessary skills needed in clinical settings such as clinical prioritization, team work and closed loop communication with seniors, colleagues and allied healthcare staff. I am competent in essential inpatient clinical procedures and currently seeking opportunities in a supervised RMO equivalent medical role. I am eligible for AHPRA registration under the competent authority pathway.
Full-time employment in in-patient acute psychiatric ward:-
1) admission clerking:-Psychiatric history, Mental State Examination, physical examination, prescriptions and risk assessment.
2) Regularly reviewing ECGs, blood investigations, completing VTE risk assessment for all patients and falls assessment in cases of falls/head injury.
3) Assessing patients with pre-existing medical conditions or new onset symptoms, arranging investigations and liaising with appropriate on-call medical/surgical teams to seek senior clinical advice for complex cases and formulate a treatment plan.
4) Being in charge of managing all types of emergencies: full ABCDE assessment, starting initial treatment, providing immediate life support and facilitating immediate transfer to emergency department.
5) Addiction Psychiatry:- Treating patients with acute alcohol and opiate withdrawal symptoms along with treating all forms of deliberate self-harm behaviour.
6) Documenting ward meetings and preparing discharge summaries.
1) Completing assessment of newly transferred patients from wards, ED or Cathlab: history, physical examination, requesting investigations, and implementing post-intervention treatment plan put in place.
2) Presenting all admitted cases during consultant-led rounds and updating all consultants with any new changes in patients’ clinical condition.
3) Carrying out clinical procedures like nasogastric tube placement, urethral catheterisation, venous sheath removal, blood transfusion and blood sampling.
4) Reviewing blood investigations and taking appropriate treatment steps to correct abnormalities.
5) Being part of the emergency response team:- attending emergency NEWS calls for deteriorating patients in wards, initiating first-line treatment and facilitating transfer to ICU/CCU as appropriate, providing advanced life support during cardiac emergencies including CPR for cardiac arrests.
Full-time employment in the department of transfusion medicine and blood centre:-
Being part of weekly blood donation camp team: physical examination & selection of healthy donors, managing adverse clinical events occurring during or immediately after blood donation, assessment of all blood products by confirming and verifying cross-matching test results, additional antibody testing, molecular testing and ensuring safe and timely dispatch of these products.
Attended Home Treatment Team (HTT) meetings, Ward rounds and MDT meetings every week, presented cases during HTT meetings, learnt about MSE and rating scales and attended teaching sessions on Dementia: types, neuro-imaging, treatment and cognitive tests and Schizophrenia: first rank symptoms, diagnostic criteria, investigations and treatment & case based questions.
Full-time employment in general medical+surgical wards:-
1) Completing ward rounds and acting on treatment plans advised by consultants.
2) Performing routine clinical procedures and looking after all patients independently on night shifts and collaborating with other departments for referrals and patient transfer.
3) Keeping all consultants updated with any new changes in patients’ clinical condition.
4) Attending patients in the pre and post-operative period.
5) Maintaining documentation following ward rounds, examination, communication or procedure and preparing patient discharge summaries.
Full time employment as a PGY-1 equivalent Intern:- Rotated through all major medical and surgical specialties which helped establish foundation for future clinical practice which were as follows:
1) Internal Medicine (including emergency medicine) – 2 months.
2) Obstetrics & Gynaecology – 2 months.
3) General Surgery (including Anaesthesia, ENT & Ophthalmology) – 3 months.
4) Orthopaedics – 1 month.
5) Paediatrics – 1 month.
6) Community Medicine (rural and urban settings)– 2 months.
6) Psychiatry – 1 month.
