I am a dedicated therapy assistant with over 17 years experience working in a Community Hospital (inpatients) during this time I have gained a wealth of experience with patients with various medical/orthopaedic/neurological conditions.


I have also gained knowledge of working in the acute hospital for the last three months.


I am a good team player, use my initiative and can manage and prioritise my own work load.


I am a good time keeper, listener, I sympathise and have empathy with patients, families/carers.


I will seek help if unable to answer appropriate issues from a professional.


I strive to deliver high quality of care with compression, respect and dignity.


I strive to improve patient care to motivate and organise others to ensure best practise by being happy, approachable and actively involved within my outcome for the patient.


I work with the multi-disciplinary team and have a good understanding of roles we each have to achieve the best outcome for the patient's care needs, reaching their goals and are medically fit for discharge including referrals onto relevant agencies.

Check pt observations before and during treatment if required escalate to the therapist, medical/nursing staff.


Undertake mobility/trsf and stair practise individually as instructed or jointly with the physiotherapist.


Monitor and record patients progress and reporting back to physiotherapist.


Record accurate in patient medical notes using S.O.A.P format.


Gather home environment details with patient, family or carers. Liaise information back to therapists.


Organise equipment that has been assessed as appropriate, check equipment is adjusted and used correctly by the patient.


Attend mandatory and in service training.

In my role I gather medical and social information ready for the therapist to complete their initial assessment.


Physiotherapy - I undertake exercise programs as prescribed either individually or in a group, transfer / mobility and stair practise. Or jointly with the therapist when required.


OT - Practise activities of daily living such as wash/dress or kitchen, teaching use of equipment eg dressing aids.


Gathering home environment details either face to face, telephone or emails.


I observe patient before and during treatment and escalate any issues and concerns with the therapist, medical/nursing staff.


Discuss outcome of patients treatment with therapist to plan ongoing plans/goals.


I organise equipment that has been assessed as appropriate, check equipment is adjusted and used correctly by the patient. I check stock levels, follow up on orders.


I attend MDT meetings to represent OT/PT as required


In my role I use various IT programmes Microsoft excel, word, e-rosta and RIO to record clinical notes using S.O.P/S.O.A.P format.


Clerical duties eg photocopying exercise programs, inputting data on excel, up dating handover sheets under the instruction of the qualified therapist.
