  • I attend the weekly MDT to discuss clinical and social issues with the extended primary care team, comprising GPs, district nurses, familiar faces team and clinical psychologist, mental health practitioner, physiotherapist and social prescriber.
  • Follow the SEA monthly meetings minutes and contribute and SEA as required.
  • liase appropriately with the wider practice tea, including the administrators and reception team.
