Triage and Assessment Team -
- Conducted triage calls, assessing whether someone would be best suited to CAMHS or external services. I am then responsible for making the relevant referrals and/or implementing safeguarding measures to ensure each young person receives the level of care they require.
- I assisted the band 6's in conducting mental health assessments of young people to see whether CAMHS is best suited and if so, which part of CAMHS they should be referred to.
- I also conducted a weekly assessment independently, following the process of referral or discharge as appropriate.
- Compiling care plans, risk assessments and safety plans.
- Managing risk accordingly and escalating risk as appropriate.
- Liaising with MDT's to collaborate on tasks to best support an individual.

Prior to the development of the triage and assessment team -
- I successfully held a caseload of 27 young people in which I was responsible for co-ordinating their care.
- Attending medication reviews and escalating medication queries appropriately.
- Conducting home visits for those who were most vulnerable.
- Following episodes of care throughout leading to the successful discharge of quite a few of my young people.
