Skilled Care Provider assisting elderly, convalescents, or persons with disabilities with daily living activities in-home or within care facilities. Keep house by making beds, doing laundry, washing dishes and preparing meals. Advise families and healthcare team regarding health concerns, nutrition, cleanliness and household activities.
· Deliver personal care (washing, dressing, toileting, etc.) to clients with complex health needs.
· Support with clinical tasks such as tracheostomy care, ventilator use, PEG feeding, and medication administration.
· Monitor and report changes in clients’ physical or emotional condition.
· Use equipment safely (hoists, wheelchairs) for mobility and transfers.
· Keep accurate care records and follow care plans.
· Provide companionship and emotional support to clients and families.
· Work closely with nurses, therapists, and other healthcare professionals.
· Follow infection control, health & safety, and safeguarding procedures.
· Participate in ongoing training for complex clinical tasks.
· Work flexible shifts, including nights and weekends, as needed.
