This was a totally new service - the ethos was to provide a hospital quality service to a patient in their own home. All the members of the team moved into the community and we had Health care assistances, Trained nurses, advanced Nurses, GP's, OT's, physio, dietitians, podiatry, SALT social workers and pharmacy team.
Are aim was to prevent hospital admission and to support early discharge from hospital and in the beginning it was a first class effective service.
As the only pharmacist for the team I lead all the Procedures for medications and was also asked to support the community Out-patient IV Antibiotic Team.
I was involved in reviewing and and writing polices with regards to medicines including parenteral bisphonates in the community; Intravenous antibiotics and infusion fluids. I also developed training polices for administration of mediacies by health care assistances..

I was a member of the harm free team and would investigate medication incidents and would make recommendations to improve systems that would reduce the risk medicine errors
I continued my work in Intermediate Care and the service grew and during my time I completed my prescribing course.
I was also acted as the deputy manager for the the dispensary and the aseptic department and was involved in Total Parenteral Nutrition (TPN)
Unfortunately due to the new build of the new Royal Liverpool Hospital the Intermediate care wards needed to close and they decided to move into the community to provide a "hospital at home service" I was then TUPED to Liverpool Community Health

I started as a care of the elderly pharmacist and developed my knowledge of fragility.
The hospital also had in-patient ward for dermatology which was very interesting.
The hospital also hosted an Intermediate Care Ward, were I became the lead pharmacist for these wards. It was a Nurse Lead Unit with GP cover and I found my role very rewarding.
Nurse and Pharmacist Independent Prescribing was gathering momentum and I took a lead role in supporting Nurses to become confident advanced practice nurses.
While at Broadgreen it was decided that Broadgreen Pharmacy should merge with Liverpool Heart and Chest Hospital.
I was also the deputy dispensary manager whilst at Broadgreen and ensured the work flowed well
I was also a mentor for the pre-registration pharmacists and junior pharmacist

During my employment I developed my skills in caring for neonates which was fascinating.
I was also an integral member of the Palliative MDT for Gynaecological Centre and developed my passion to provide to provide the best care and pain relief in the last days of life.
Whilst at the Women's I was a diploma Tutor for for a pharmacist for a junior pharmacist during her clinical diploma

Fourth Valley was a huge Mental Health Trust spread over a large geographical Area I worked on several psychiatric wards.
My main role was as an interface pharmacist between Primary and Secondary Care ensuring that patients in the community on secondary care meds received their meds without any problems.
While there the country implemented the Meningitis C Vaccine campaign and I was the lead pharmacist for that region for the campaign.
