A highly motivated and experienced Clinical Pharmacist currently working in a GP surgery in Swindon, I am committed to improving patient care and health outcomes within a multidisciplinary team. With extensive experience in utilizing the BSW formulary and adhering to NICE guidelines, I have developed strong skills in ensuring safe and effective medication prescribing. My previous role with the Bath and North East Somerset, Swindon, and Wiltshire ICB allowed me to focus on cost-saving initiatives for the NHS, leading to cost-effective medication switches and driving quality improvement projects such as STOMP and STAMP. I also have experience in piloting clinical searches and analysing data to support GP practices. During my pre-registration year, I honed my ability to communicate effectively within a multidisciplinary team, ensuring that changes or issues were promptly relayed to the relevant team members. I understand the importance of documenting my work as a healthcare professional, particularly when I shared the responsibility of managing an admission ward with another pharmacist. My clinical ward rotations in various specialties, including cardiology, respiratory, geriatric medicine, stroke, neonatal care, and intensive care, provided me with the confidence to clinically screen a wide range of patients under pharmacist supervision. These experiences taught me to recognize my limitations and seek support when needed. I relied on resources such as local and national guidelines, NEWT guidelines, the renal drug database, and the Electronic Medicines Compendium (EMC) to support my decision-making and identify errors, including medication conflicts with guidelines, allergies, weight, renal function, or inaccurate medication histories. Through my pre-registration training at Great Western Hospital, I gained valuable experience working with systems like Nervecentre, JAC, and EPMA to optimize patient health outcomes in secondary care. I am now seeking new challenges to further develop my clinical skills and contribute to enhancing healthcare delivery.
