  • Lead elements of the Council’s preparation for the new statutory duties included in the Domestic Abuse Bill
  • Ensure that Devon’s Community Safety Partnerships carry out Domestic Homicide Reviews that produce and embed effective learning
  • Commission, brief and oversee progress of Independent Panel Chairs/Lead Reviewers including negotiating fees, agreeing payment schedules and approving claims.
  • Managing the Domestic Homicide Review Co-ordinator and providing support and appropriate supervision
  • Carry out an impact assessment of the reviews published to date and identify recommendations for the future governance and resourcing of these reviews.
  • Support the implementation of recommendations resulting from reviews conduct an audit into their impactfuliness across the region
  • Lead on work with other Domestic Homicide Leads, CCGs and across the peninsula to find ways for joint working to create a more effective efficient and sustainable model


Key Achievements


  • Working with the Head of Domestic Homicides for the Home Office as well as the Domestic Abuse commissioner on how they can improve the process for reviews and template following raising major problems with the current process.
  • Leading a peninsula wide approach to DHRs to help make them more efficient
  • Improving the current internal DHR process and looking to improve the average DHR completion time down from two years back to the six months which is stated within the guidance. including incorporating more stake holders earlier on in the process and ensuring wider sharing of learning across the region and wider
  • Bringing together an impact audit on the recommendations and actions since 2012 to analyse how these have evolved over the years and then working with stakeholders to understand how much impact these have had and whether they are working. This helped us understand which workstreams your under the most pressure and meant we could focus on those areas which needed it most.
  • Working with the leads for Safeguarding Adult Reviews and Child Safeguarding reviews to ensure consistency across the departments.

  • Management of patient safety assurance processes and oversight of all serious incidents across Somerset, including those outside of the region where Somerset patients are involved.
  • Monitor the NHS Trusts and Providers involved in serious incidents to make sure cases are reviewed in line with the National Framework and take appropriate action when they are outside of timeframes.
  • Produce and present monthly, bi-monthly and quarterly presentations to upper management, reporting data summaries I have analysed regarding serious incident cases.
  • System manager for the Datix Risk Management System database, reports, analysis, action plans and learning for individuals, directorates and the Trust on all types of incidents.
  • Support and manage the entry, reporting and development of the CCGs corporate and directorate risk register.
  • Deputy Local Area Contact for the Learning Disabilities Mortality Review (LeDeR) Programme in Somerset.
  • Lead for the Domestic Homicide process for Somerset CCG, which included information gathering producing the initial report, attending the overarching meetings, working closely with the Local Authority and implementing the actions required
  • Undertaken a national review of root cause analysis reports with a view of standardising the report template across all Trusts (work with the Head of Investigations for the NHS)

Key achievements/projects


  • Undertaken and implemented a review and re-work of internal procedures and policies, including updating and completing the new Serious Incident Policy.
  • Improve performance across the region from the NHS Trusts/providers in line and against the National Framework.
  • Setting up tracking systems for monitoring completion of actions in relation to Safeguarding children reviews, adult safeguarding reviews and domestic homicide reviews.
  • Reduced the number of outstanding risks to review on both the Directorate and Corporate risk register.
  • Supported the implementation and delivery of charge through collaboratively working across the country with partner agencies to help prevent future occurrences of incidents.
  • Wrote and produced Somerset biggest Multi-agency Serious Incident to date which involved every agency in the area, I had to co-ordinate multi-agency meetings, gather all information including meetings with family, present the findings and report, fed the report back to the family at the end. Report was supported by Dr Jonathan Leech, who approved the suggested changes and agreed they required national implementation as laid out

  • Produce weekly and quarterly reports and a monthly maternity report to present to the Quality and Safety Manager and Deputy Director of Nursing.

Key Achievements/projects


  • Taking on the role of reviewing the Primary Care (GP, Dental and Pharmacy) incidents on top of my usual work load, due to my experience and knowledge of the process.
  • Presented and implemented new SI processes for the organisation, which provided further and stronger governance, assurance and robust decisions surrounding SI's and therefore improving efficiency.
  • Improved performance across the region by linking in with CCGs and Providers.
  • Decreased the number of primary care incidents which were declared SI's.
  • Created a regional spreadsheet to monitor the implementation of change across the south west to ensure the timeframes were met and therefore helping prevent future harm.
