Rotational Neuro Physiotherapist

Oxleas NHS Foundation Trust
Woolwich, Greenwich
2025.11 - Current

As a Community Neurological Physiotherapist, I specialize in rehabilitating patients with neurological conditions, particularly stroke survivors and individuals with Parkinson’s disease. My role focuses on facilitating safe transitions from hospital to home through the Early Supported Discharge (ESD) pathway, ensuring continued rehabilitation to enhance independence and quality of life.

I conduct detailed neurological assessments to identify mobility and functional impairments, creating tailored, evidence-based rehabilitation programs. Interventions include gait re-education, balance training, and falls prevention strategies. Strong clinical reasoning and adaptability are crucial as I deliver care within patients’ home environments, monitoring progress and adjusting treatment plans accordingly.

Effective caseload management is essential, allowing timely interventions for patients with varying neurological complexities while ensuring continuity of care. Interdisciplinary collaboration with occupational and speech therapists, nurses, and rehabilitation assistants is vital for coordinated, patient-centred care.

Additionally, I mentor physiotherapy students and have organized a cardiovascular rehabilitation group for patients with complex needs. My experience has honed my skills in neurological assessment, community rehabilitation, and holistic, patient-centred care, adapting interventions to individual circumstances.

As a Physiotherapist in a community reablement service, I provide rehabilitation to patients after hospital discharge or functional decline, focusing on those recovering from falls, fractures, and surgeries. My role emphasizes regaining strength, mobility, and confidence to maintain independence.

Managing a complex caseload, I prioritize and autonomously assess patients' needs, identifying risks and implementing physiotherapy interventions. I conduct comprehensive assessments and create individualized rehabilitation programs, including exercises for strength, balance, and mobility within the home.

Collaboration is vital; I work with occupational therapists and various teams to ensure coordinated care and safe discharge planning. I am also developing my leadership and educational skills, training to become a clinical educator, which reflects my commitment to professional growth and supporting colleagues.

This role has enhanced my skills in case management, risk assessment, and providing holistic, patient-centered care tailored to individual needs and environments.

MSK physiotherapist

Berkshire Healthcare NHS Foundation Trust
Tilehurst, West Berkshire
2025.03 - 2025.09

As a Rotational Band 5 Musculoskeletal (MSK) Outpatient Physiotherapist at Physiolistics in Reading, I provide comprehensive, evidence-based physiotherapy to a wide-ranging patient population, aged 11 to 80, who present with various musculoskeletal conditions, including acute injuries, post-operative rehabilitation, and chronic pain. I have developed strong competence in recognising and appropriately managing red flag presentations including suspected Cauda Equina Syndrome, Vertebrobasilar Insufficiency and Cervical Myelopathy, ensuring prompt clinical decision-making, appropriate safety-netting and escalation in line with local and national pathways. I am skilled in conducting detailed assessments and devising individualised treatment plans that combine manual therapy, exercise prescription, and patient education, with the aim of optimising functional recovery and promoting long-term self-management.

Patient education and empowerment are central to my clinical approach. I work to ensure that each patient has a clear understanding of their condition and is equipped with the tools necessary for self-management. My communication is adaptable to meet the needs of diverse patients, whether I am guiding a young athlete through their rehabilitation or supporting an older individual in managing a long-term musculoskeletal condition. I prioritise clear, compassionate, and patient-centred communication to foster trust, independence, and confidence throughout the rehabilitation process.

In my role, I collaborate closely with a multidisciplinary team, including senior physiotherapists, rehabilitation specialists, and external consultants such as members of the IPASS and CSS teams. This multidisciplinary approach enables me to provide coordinated and holistic care, particularly in managing complex cases, and ensures that patient care is aligned with the latest clinical guidelines and best practices.

I am committed to ongoing professional development and actively participate in in-service training to stay at the forefront of current musculoskeletal physiotherapy practices. In addition to clinical care, I support the education and development of physiotherapy students by providing informal teaching, mentorship, and guidance during their placements, ensuring they are able to apply theoretical knowledge to clinical practice effectively.

I maintain the highest standards in clinical documentation, confidentiality, and professional conduct. I also contribute to service improvement initiatives and clinical audits aimed at enhancing patient outcomes and optimising service delivery. My practice is grounded in delivering care that is both evidence-based and patient-centred, ensuring that I consistently meet the professional standards required to provide safe, effective, and compassionate care.

I am driven by a genuine passion for physiotherapy and a commitment to making a meaningful difference in the lives of my patients. Through continuous learning, collaboration, and patient-centred care, I strive to uphold the highest standards of clinical excellence and contribute positively to any team I am part of.

Inpatient Physiotherapist

Berkshire Healthcare NHS Foundation Trust
Tilehurst, West Berkshire
2024.09 - 2025.03

In my role as a Physiotherapist in an inpatient reablement community hospital, I am responsible for delivering high-quality rehabilitation to a diverse patient population. I assess, treat, and manage individuals recovering from post-operative conditions, neurological impairments (stroke, brain injuries), amputations, and dementia-related mobility challenges. My ability to provide patient-centred, evidence-based care has contributed to improved functional outcomes, enhanced independence, and reduced hospital readmissions.

Caseload management, prioritisation, and delegation are essential aspects of my role. I efficiently manage a high-demand caseload, ensuring patients at risk of deterioration receive immediate intervention, while maintaining rehabilitation progress for others. Through effective delegation within the multidisciplinary team (MDT), I ensure streamlined ward operations and optimal patient care.

A key focus of my role is conducting comprehensive patient assessments and developing tailored rehabilitation plans. I perform functional mobility evaluations, gait and balance assessments, strength testing, and transfer training to create personalised treatment strategies that promote independence and confidence in movement. I am skilled in manual therapy techniques, exercise prescription, and mobility aid recommendations, ensuring patients receive the most effective interventions to support their recovery.

Discharge planning is a critical component of my role. I work closely with occupational therapists, patient flow coordinators, discharge coordinators, and social workers to ensure that patients transition safely, efficiently, and with appropriate rehabilitation support. I assess home environments, arrange for adaptive equipment and community rehabilitation, and coordinate ongoing care, reducing the risk of readmission and ensuring a seamless transition into the next stage of recovery.

Interdisciplinary collaboration is at the heart of my practice. I actively participate in MDT meetings, discussing patient progress, rehabilitation goals, and discharge planning. By working closely with nurses, consultants and occupational therapists, I ensure a holistic and coordinated approach to patient care. My ability to understand the roles of different healthcare professionals allows me to delegate tasks effectively, enhance teamwork, and contribute to well-informed clinical decision-making.

Beyond direct patient care, I am passionate about teaching and mentoring. I support physiotherapy students and junior staff, providing structured clinical training, hands-on guidance in assessment techniques, treatment interventions, and rehabilitation strategies. This experience has strengthened my leadership, communication, and reflective practice skills, ensuring I contribute to the professional development of future physiotherapists.

I am committed to delivering evidence-based physiotherapy interventions that enhance patient function, safety, and long-term well-being. My approach integrates exercise therapy, functional rehabilitation, manual therapy, and patient education, ensuring that each patient receives a comprehensive, individualised treatment plan. Through timely and accurate assessments, I adapt interventions dynamically to align with patient progress and rehabilitation goals.

This role has deepened my passion for rehabilitation and strengthened my ambition to develop further within a rotational physiotherapy position. I am eager to refine my MSK skills, expand my expertise in neurological rehabilitation, and gain broader clinical exposure. The structured training and diverse rotations at Maidstone and Tunbridge Wells NHS Trust will provide the ideal platform for my continued professional development, allowing me to contribute my expertise, dedication, and commitment to delivering high-quality physiotherapy to your team.
