Job overview

My 1st training job as locum appointment.I consider this job a golden opportunity to work on my skills and to learn new skills.As a supervised trainee I was introduced to many exciting opportunities for my career.This is a busy hospital with multiple subspecialties, hence busy place to work.At end of every work day, I have learned something new and added value to my CV.My interest is in UROGYN and I got good chance to attend urogyn theatres and also got chance to perform some of supervised surgeries. Keilland Forceps a wonderful but limited art,I got a chance to practice here.Gyn. Onc & Fetal Medicine are also very busy here and as part of team ,1st time while working in OBGYN I worked with gyn oncologists.I attended robotic surgeries and have experience of assisting robotic surgeries.I came across many complex cases in both gyn & Obstetrics in this hospital as it is hub of OBGYN for a wide area and also multicultural and multinational communities reside here.There is long list of diseases which I read in books only but I had to manage and treat here as part of my team.I taught medical students in the university of Dundee and played role of examiner as well.I can proudly say, this post added value to my work experience.

I worked in this trust as senior clinical fellow.This was my 1st appointment where I started working on E-Portfolio and sort of introduced to more aspects of progress in OBGYN as a middle grade doctor.I worked as 2nd on-call during OOH and had consultant support from home.I worked as 1;8 rota.I was given more registrar Gyn clinics and was supported by consultants if needed any help.Same for ANC ,where I was actively involved in ANC management plans with consultant support and my most of clinics were with maternal medicine consultant.I learnt a lot from this trust and was one of active and responsible team members.I led Obs elective CS list with support of on-call consultant.My 1st appraisal also done here .I had exposure of London vibes here.England was different than NI.I must appreciate people here are familiar with IMGs and welcome generously.I regularly attended perinatal & risk management meetings and presented cases.I was organising PG teaching in department.Hysteroscopy & laparoscopy basics I started in this hospital.I had good reputation with my colleagues.In this area patients style was different.I found them bit challenging.But I took this as an opportunity to improve my communication skills and I succeeded in this as per feedback.

Job overview

This was my 1st job in UK. Daisyhill was small hospital but with very friendly and welcoming staff.I was lucky to have best trainers there who were very generous to teach.This post was not just a change of job for me but a huge change in my life style, culture ,language,workstyle. When I started ,I had no idea of UK system(except theoretical knowledge for MRCOG).I got familiar with midwifery & primary health care services which was a new concept for me.I learnt physiological CTG 1st time in this trust and found it easy to interpret and manage patients. I was involved in Audits & QIP .I was running EPAC clinic under consultant supervision.Got signed off for instrumental deliveries and 3rd degree tears.I actively participated in PG teaching in department.

I was very comfortable there and was offered permanent post but I decided to explore further improvement options in career and that was small hospital with limited options so I decided to join more busy hospital.

AUDIT #1

Title.....Cooks balloon use for IOL(change in amount of saline used to fill uterine balloon) conducted 14/01/2024

       I observed in unit,number of cord prolapse CAT1 sections has been increased after use of cooks balloon for IOL .So I conducted an audit under supervision of consultant by retrospective data collection for 1year before and 1year after introduction of balloon for IOL.

      Conclusions/Actions.  Number of cat1 cs after IOL with balloon has increased 3times.ACTIONS:Confirm Presenting part&engagement abdominally before starting induction&to avoid induction in high or less engaged head.To limit amount of saline upto 40mls used for IU balloon.As per studies its impact on elevating head is less.


AUDIT #2 

Title......GBS guidelines compliance in SHSC NI conducted on 09/04/2021

       This was a PHA audit which i conducted with 1 of my colleagues.and found our trust was 100% compliant with guidelines except one point which was absence of sticker on file.

         Conclusions/Actions. Conclusion was our trust was compliant with GBS guidelines of NI except missing GBS stickers on positive pts files.

         Action taken was to make sure ,presence of stickers on all +ve pts files.

I believe every audit or change is valuable as its comparison to standard guidelines.
Personally I enjoyed Balloon impact on cord prolapse or elevating head out of pelvis.There are no clear guidelines about this so it was more challenging and results were quite impressive. This idea came in my mind while filling a balloon in IOL and I thought 80mls could be equal to a small baby head size ,so are we doing right thing by pushing head up and when I worked on it ,I found my thought had value, so this gave me confidence.

        NOTE.  I have more audits to show as well but to avoid long CV adding just 2.



Teaching

Teaching

I have been formally teaching medical students in The University of Dundee (Y2 &4). I taught them in groups of 6-12 with topic discussion followed by question answer sessions. I collected their formal Feedback in written form to improve my teaching skills and applied positive points in my teaching. I have a good record of measurement of teaching from University.
I have been teaching and training students in OSCE preparations.I was awarded certificate by local gyn students society, for helping them preparing for exams.

I am formally trained as OSCE examiner and certified for that by The University of Dundee. I have participated as examiner for medical students OSCE exams for year 3,4&5.
I have been Faculty of PROMPT course.
I have done train the trainers course as well.

Management and Leadership Experience

I have good management and leadership skills and I keep practising my skills in both personal and professional life.I am very proud to say I have formally collected some good feedback from my team to show in my portfolio.I,ll list some of examples of my projects where I played role of team leader,
1. I am rota writer of middle grade doctors. I managed this role by keeping in view training & learning needs of trainees and other trust doctors as well.I tried to give equal chance to everyone to learn new skills and attend important meetings .I kept asking for team feedback and their needs to help everyone.

2. I had opportunity to take leader role in practical emergency senarios including obstetrics emergencies like PPH,APH,Cord prolapse, maternal collapse and managed very well and received good feedback from my team and have proof of appreciation in form of formal official letter too.
3. I am organising PG teaching in department along with another registrar, and make sure everyone is able to attend and ask questions.
4.I am one of the faculty members of PROMPT and lead simulation tasks during course.

5.I am leading a charity organisation to help needy people in my home town and special interest is to educate children and help them establish their careers. This started 3years ago and is working effectively so far.Looking forward to see people improving their life standards.

I would like to quote here most recent situation where I was on call registrar in LW. A post CS patient in post op area started deteriorating clinically .Attending MW called for help. Myself,SMW,Anesthetist attended patients and I clearly announced I am taking lead role and gave tasks to other members by their names and made sure command was clear and loud.We started evaluating.Meanwhile asked for more help&once anaesthetic consultant arrived,I handed over to him lead role by saying it clearly and loudly .

I confirmed my commands close loop communication because it makes things smooth and easy to carry on.
Patient was not getting any better and had to have intubated in Post-op observation area but till that time we ruled out cause of maternal prolapse and were ready to return to theatre with everything in place .All team members were quite efficient and patient returned to theatre and reopened where intra-abd haematoma found and ligated bleeding vessel. In start our suspicion was acutely worsening sepsis vs intraabdominal bleed but at that point Hb and USS didn't show any signs of bleeding.However as a team we kept working on cause and soon found and treated cause. Patient had very good recovery. learnt a lot from this event and I was impressed by my team members.Clear and close loop communication and nominating persons for tasks is helpful to run task smoothly. Our workfield is very sensitive&good outcome is result of good teamwork.
