  • Preparing the rota to meet the needs of the clients and where necessary altering them to accommodate any changes that may arise due to sickness, leave or change of client's activities.
  • Preparing and reviewing residents care plans, risk assessments person - centered plan (PCP) report with them where possible for their review meetings.
  • Inviting and chairing their PCP meetings as agreed with them.
  • Where residents are not capable due to lack of ability, then I liaise with other relevant people involved in their care to decide what's in their best interest.
  • Ensuring such assessments are done following the mental capacity act 2005 and the relevant documents every time an intervention is needed.
