I have worked in Social Care since 2008 with various companies in various settings and with many different client groups.
I have experience working with older people in residential care, adults with varying degrees of physical and learning disabilities, adults with enduring mental health issues.
All at times presenting with challenging/stress/distress behaviours and having varying degrees of complex
issues and support needs.
As a support worker with in this organisation my main duty was to ensure that service users received the necessary support and assistance to meet their needs.
To do this I used various assessment tools along with service users to ensure we agreed on what type of support was required and how we would implement this support, this also allowed me to ensure appropriate interventions from ourselves and other agencies where included as part of the care package.
I was also responsible for developing, maintaining and evaluating comprehensive support plans.
It was also important to establish, build and maintain therapeutic relationships as well as working along side other agencies.
I was also able to support and assist service users to participate and enjoy activities of daily living.
At times this involved teaching new daily living skills or revisiting skills, e.g. cooking, budgeting skills and shopping.
I worked closely with my line manager and peers participating in activities such as team meetings, in house training and supervision. Also providing updates or necessary information when required to my peers, line managers and external managers.
I worked closely with other agencies involved in service users support and care and again built and maintained good working relationships.
I was fully aware of my role and responsibilities with regards to my own and others H&S. This included reporting incidents to my immediate
line managers, external managers and other agencies.
I was able to demonstrate my knowledge of the Health and Social Care Standards in my practice and worked within the organisations
