I qualified as an Adult Registered Nurse in 2011.

My passion and interest has always been delivering care to my patient in a Community and Clinical setting within the NHS Service.

I have acquired many clinical skills and attributes that can be transferred in a Practice Nurse Setting.

I am excited about the prospect of starting a new career in Primary Care and look forward to working in new clinical environment and learning new skills.

I have good local knowledge of South Derbyshire due to previous work experience as a Community Nurse.

I am hardworking, friendly and approachable and can be flexible to all shift patterns that are required.

  • I am responsible for the overall Caseload Management of patients that are registered with GP Practices within the Amber Valley Locality.
  • Monitor standards of care, deploying and supervising team members as appropriate and the daily management of own workload. I am currently responsible for seventeen members of staff that include registered nurses, nursing associates and health care assistants.
  • Co-ordinate the assessment, planning, implementation and evaluation of care programs which reflect the individual needs of patients and carers.
  • Plan, implement and evaluate a learning contract for student nurses involving multi-disciplinary agencies. I work closely with GP Practices providing insight days for the Practice Nursing Teams.
  • Be involved in the education and training of newly qualified nurses and other staff employed within the Trust
  • Have the initiative to respond to challenging and unpredictable situations/behaviours that arise in the clinics.
  • Manage any complaints, errors, concerns and other untoward occurrences in line with local policies, procedures and guidelines. This includes issues relating Safeguarding of Vulnerable Adults and Children.
  • I am aware of my limitations and know when to escalate clinical or operational issues to my direct line manager.
  • Co-ordinate the assessment, planning implementation and evaluation of Care Plans that reflect the individual needs of patients that meet best practice using advocated tools/systems as required.
  • As well as the management duties I also spend time in clinic, delivering a variety of wound care techniques such as compression therapy, wound packing, VAC Therapy, education and support and promoting self-care as much as possible. I work closely with other Specialisms such as Vascular/Lymphoedema/Dermatology/Podiatry Departments. Vital Observation monitoring, NEWS score and escalating clinical concerns appropriately.
  • I am registered with SystmOne and use this system to document confidential information in regard to the patients medical information.
