• Working as an autonomous practitioner, providing confidential, non-judgemental, high levels of patient centred care both face to face and remotely, and across various sites.

Manage and coordinate the requests for spoke placements for pre and post registration students wishing to spend time with SHW, and to liaise with others agencies both within the trust and external partner agencies to identify and provide appropriate learning experiences and spoke days for pre-registration nurses placed within SHW.
• Manage the electronic results, including investigating and resolving sample/lab errors. liaising and discussing with relevant staff to address clinical errors, to minimise future occurrences. This includes aspects of 'informal auditing' of patient records. Contact with and recall of patients when appropriate. Informing patients of their results and discussing appropriate management and treatment plans.
• Discussing sensitive diagnoses and situations such as HIV diagnoses/sexual assault/exploitation, in a compassionate manner, whist maintaining patient dignity.
• Auditing of patient records, and feeding back outcomes to the wider team.
• Delivering non-judgemental support and services to individuals under the age of 18 utilising Fraser Guidelines and ensuring that the young person is aware of the legal aspects of their care and referring for appropriate safeguarding when appropriate.
• Nominated Link person for several groups of patients and external agencies in the local area; MenCap Wirral, Tomorrows Women, and Wirral Ways to Recovery.
• An inclusion champion and champion the needs of the disabled and LD community that access SHS.
• Take the lead in Partner Notification and particularly with Provider referrals.
• Act as the Clinical Lead for the service Outreach Team, working collaboratively with our GP Champion and Service Development coordinator to deliver Health Promotion sessions for service users, and partner agencies, and also other medical and health professionals.
• Currently sit on the Faculty of Sexual and Reproductive Healthcare Expert Working Group Committee helping to develop evidence based training and educational resources for the FRSH membership, and wider community.

Supervision and support of both junior members of the clinical team and admin. Management of clinical results. Clinical lead for Health and Safety. Safeguarding Link Nurse. Supervision and training of junior staff where required. Delivery of health promotion sessions for service users and partner agencies. Delivery of Outreach Services in various venues such as LARC/contraception provision in Women's refuges and hostels, CLA supported accomodations.

Work autonomously to deliver all aspects of the Healthy Child Programme to an identified case load of children and families. Manage own time and caseload. Train, support and assess Specialist Community Public Health Nurse trainees. Liaise closely with school staff to ensure any concerns addressed in a timely and appropriate manner. Attend safeguarding/child protection/child in need meetings as required. Complete CLA health assessments for those young people on my caseload. Ongoing support and liaison for children, families school staff as required. Health promotion in Schools for students. Health education sessions for Staff re: anaphylaxis management. Coordinate the delivery of the school aged vaccination programme in identified schools, including 'mop-up sessions'. Maintain accurate and contemporaneous child health records, and provide medical reports where appropriate. Liaise with other members of the 0-19 health team where required. Provide confidential 'drop in' clinic service available to children/young people/parents/carers school staff needing support and advice. Co-ordinate and deliver specialist Enuresis clinics.

Deliver patient centred holistic care for an identified group of patients. Act as nurse in charge when required. Provided mentorship and supervision of student nurses and other junior members of the ward team. Liaise with the medical team/families. Assist with Activities of Daily Living when required.
