in the Nottinghamshire area.


Intensive support offered includes :

* Initial assessment in order to create a person centred support plan. Reviewed bi-monthly.

* Weekly support sessions via telephone, video link or face to face in the community.

* Extra support sessions if requested/necessary (for example appointments, move in dates)

* Support to prevent homelessness if possible (eg sustaining a tenancy, paying rent arrears, eliminating risks such as hoarding) or referrals sent to hopefully find appropriate/suitable accommodation.

* Advocating for the individual whilst attending MDT meetings, finding accommodation and supporting with an approach to the local authority for a homelessness assessment/housing application.

* Support to move in ( eg Moving personal items, tenancy sign up, setting up utility bills, GP registration, furniture project referral, food parcel, DWP address update or application, involved agencies updated, budgeting lists)
