Transitions Officer working in partnership with mainstream and special schools, families and external agencies to support young people aged 14 to 19 with learning disabilities and or autism as they moved from school into further education, training or employment. Coordinated person centred transition planning, provided guidance on appropriate pathways and ensured continuity of support, helping service users build confidence, independence and realistic progression routes beyond school.
