Working within an A&E setting. Report to shift coordinator for allocation. Assessing and providing emergency treatment and care for acutely unwell patients. Prioritising time critical treatment/medications. Including, management of unstable cardiac condition's,trauma events, sepsis, burns, respiratory conditions, trips & falls. Monitor of mental health, drug and alcohol abuse patients for their safety and deterioration.
Deliver life support with up-to-date CPR training. Assessing and choosing suitable treatment for post operative and trauma wounds. Venepuncture, Cannula insert, Catheterisation, Monitoring vital signs of patients and escalate deterioration.
Work closely and communicate effectively with MDT. Manage patient flow within the department. Keep documents up-to-date. Record by using Trusts IT systems .and hand written medical notes. Adhere to Trust policies.
Visit local hospitals to identify early discharge patients meeting criteria for home care treatment of Intravenous antibiotics.
Coordinate care across Halton community, based on patient clinical priorities. Visit patients in their own home to administer I.V antibiotics via PICC lines, Porta-Catheters, Long lines.
Covering a variety of health conditions with suitable antibiotics. Including, Infective COPD, diabetic foot/leg ulcers. Post operative wound infections to name a few.
Adhere to ANTT. Management of PICC lines, Porta catheters and long lines for dressings and troubleshooting. Assess patients health condition. Interpret blood results act upon findings if required. Record visit details on I.T Trust System. Communicate with nurse team lead and MDT. Keep patient up-to-date with their care plan. Adhere to Trust policies and pathways.
Coordinate care across Halton community based on patient priorities and caseload. Visit house-bound patients in their own home to deliver variety of care/treatment. Including, leg ulcer evaluation/treatment/dressing changes. Leg compression bandaging, Pressure sore management. Administration of Insulin, Catheter care, Palliative care. Follow Trust policies. Keep up-to-date with evolving treatments.
Coordinate care across Halton community based on patient priorities.
Working within the Mersey Regional Burns Unit. Management of critical- acute burns and on-going care. Daily re dress/assess and evaluate plan of care. Manage care of post-operative skin graft patients. Fluid resuscitation, RBC transfusions, promote mental health and well being.
Manage burns clinic to review post operative burn wound and treat patients in clinic. Discharge when appropriate. Refer to physiotherapy, tissue viability nurse. Keep up to date with new burns practice.
