2. CARE PLANNING AND DOCUMENTATION
*Updating and reviewing the care plans and the risk assessments of each resident each day.
*Checking the fluid intake of the residents and increasing the fluid intake as per the needs.
*Checking the MUST Score and sending the diet referrals and starting the supplements as suggested.
*Ensuring that the care plans reflect the needs of the residents.
*Taking the wound pictures and doing the wound care plan.
*Doing Audits assigned by the Manager.
3. MEDICATION ADMINISTRATION AND WOUND DRESSING
*Administering medication as prescribed and as per the Emar policy.
*Administering the Controlled Drugs with the Nurse.
*Monitoring the blood sugar level, recording it, and informing the Nurse
*Managing Hypoglycemia by administering the PRN and informing the Nurse in charge.
*Managing medication administration records and maintaining the inventories.
*Checking the safe administration of medication through syringe Drives.
*Assisting the nurse in wound dressing.
*Sending swabs and urine samples as advised by the ANP.
4. MANAGING CHALLENGING BEHAVIOURS
*Providing Dementia centered care to the residents.
*Maintaining the safety of the other residents.
*Administration of medication as prescribed to manage the challenging behaviors.
*Recording their behaviors in PCS and sending CHS referrals when required.
*Doing safeguarding and police reports for any altercations, filling incident forms, and informing the family members as well.
*Recording and updating the care plans accordingly.
5. EMERGENCY RESPONSE
*Checking Observations and doing Neuro observations, providing first aid, and filling out the accident forms.
*Calling 999 in case of an emergency, such as a fall, fracture, bleeding, unresponsiveness, head injury, or unstable vital signs.
6. COMMUNICATION
* Communicating with the healthcare professionals regarding the resident's condition, recording the care plan, and implementing it.
*Maintaining a good rapport with the family members and ensuring that their concerns are attended to.
*Regular communication with the Nurse and the Manager regarding the residents' health.
