1. GUIDING AND SUPERVISING THE CARERS
*Ensure that the residents receive patient-centered care.
*Allocating the residents and delegating their daily tasks.
* Providing guidance and supporting them in their tasks.
*Taking the Lead when doing shifts with the Agency nurse.
*Ensuring that the team members adhere to the standard care and the health and safety protocols.

2. CARE PLANNING AND DOCUMENTATION
*Updating and reviewing the care plans and the risk assessments of each resident each day.
*Checking the fluid intake of the residents and increasing the fluid intake as per the needs.

*Taking the wound pictures and doing the wound care plan.
*Doing Audits assigned by the Manager.

3. MEDICATION ADMINISTRATION AND WOUND DRESSING
*Administering medication as prescribed and as per the Emar policy.
*Administering the Controlled Drugs with the Nurse.
*Monitoring the blood sugar level, recording it, and informing the Nurse
*Managing Hypoglycemia by administering the PRN and informing the Nurse in charge.
*Managing medication administration records and maintaining the inventories.
*Checking the safe administration of medication through syringe Drives.
*Assisting the nurse in wound dressing.
*Sending swabs and urine samples as advised by the ANP.

4. MANAGING CHALLENGING BEHAVIOURS
*Providing Dementia centered care to the residents.
*Maintaining the safety of the other residents.
*Administration of medication as prescribed to manage the challenging behaviors.
*Recording their behaviors in PCS and sending CHS referrals when required.
*Doing safeguarding and police reports for any altercations, filling incident forms, and informing the family members as well.
*Recording and updating the care plans accordingly.

5. EMERGENCY RESPONSE
*Checking Observations and doing Neuro observations, providing first aid, and filling out the accident forms.
*Calling 999 in case of an emergency, such as a fall, fracture, bleeding, unresponsiveness, head injury, or unstable vital signs.

6. COMMUNICATION
* Communicating with the healthcare professionals regarding the resident's condition, recording the care plan, and implementing it.
*Maintaining a good rapport with the family members and ensuring that their concerns are attended to.
*Regular communication with the Nurse and the Manager regarding the residents' health.

• 1. NEUROLOGICAL ASSESSMENT AND MONITORING OF PATIENT CONDITION
  *The first and crucial step in unconscious patients or patients with neurological conditions is to assess their pain level and GCS scale to know their level of deterioration.
  * Assessing pupillary reactions and reporting to the Doctor for appropriate treatment.
  * Monitoring ECG and informing the doctor of any variations in rhythm.
  
  2. AIRWAY MANAGEMENT
  *Assess the saturation level and provide oxygen via a suitable mask as per the patient's condition.
  * Assist in intubation and administering controlled drugs like Morphine, Midazolam, Adrenaline, Noradrenaline, Fentanyl, etc., as per the order.
  * Assisting in tracheostomization and suctioning, use of the Ambu bag when needed.
  
  3. MEDICATION ADMINISTRATION
  *Medication administration is crucial, which includes patient safety.
  *Follow the 5 rules of medication administration while giving medicine and document as per the policy.
  *Before administering medication, it is important to know the action and side effects of the medication and explain it to the patient.
  
  4. COMMUNICATION
  * It is necessary to have communication skills which will help to maintain a good rapport with all the healthcare professionals, such as doctors, colleagues, physiotherapists, etc., and to work as a team for patient care.
  * Providing emotional support for the family members by explaining their current condition and providing them with reassurance is also important.
  
  5. DOCUMENTATION
  *Documentation is very important for the patient and the healthcare professionals.
  *Record all the observations and assessments done on the patient to determine his condition.
  *Record the medication administration, which also includes time, route, date of administration and the refusals of the medication as well.
  * Giving handover to the nurses as per the ISBAR.
  *Gain consent of the patient or the family member before performing any procedure.
  *Record the escalation of the patient's condition to the Doctor if any changes.
  * Checking the inventories of the medication and medical equipment before the handover and maintaining the records accordingly.
  
  6. SAFETY AND INFECTION PREVENTION
  *Maintain proper hand hygiene using the 7 steps of the WHO.
  *Follow 5 moments of hand washing to prevent cross infection. Do proper donning and doffing.
  *Maintaining aseptic technique during invasive procedures like Central line insertions, Drain removals and EVD insertions
  *Avoid Needle stick injury while doing blood samples and securing IV lines.
  *Handle the confused patient with care.
  * Use of restraints under the doctor's order for the patient and staff safety.
  *Prevent pressure ulcers by repositioning.
  * Implement strategies to prevent falls.
  
  7. EMERGENCY RESPONSE
  *Responding to the emergency situations promptly and with confidence.
  *Follow the emergency protocols and activate the code.
  *Assisting in performing CPR, intubation, medication administration and securing IV lines.
