· Preforming Palliative support for stable patients and families. Assessing and recognising decline in presentations or symptoms management. Liaising with senior staff, specialist and GPs for advanced care planning and support.
· Management of Diabetes - Blood glucose reading levels, observing ranges and administer insulin. I act on hyperglycaemic and hypoglycaemic episodes and contact emergency services if required. I also educate and train patients in relation to diabetes management and self-administration.
· Administered Dalteparin and hydroxocobalamin injections. Assessed wound care, provided appropriate dressing regimes, and delivered education on wound care and infection prevention. Liaised with GPs regarding swabs and medication for wound infections and performed photographic reviews of wounds to identify deterioration or healing, reporting to specialists as needed.
· I am trained to carry out soft bandaging, ABPI's to ensure compression is suitable on following a lower limb assessment. I deliver Food first education around the role of nutrition in healing and prevention as well as liaising with skin integrity teams including Tissue Viability and Lymphedema services. I assess any pressure areas, raising this on trust incident and auditing system as per local protocols, educate on repositioning, identify and order pressure relieving equipment.

· Dressing wounds per care plans set out via planned case managers, urgent wound swab and escalated to GPs and request medication where indicated. I liaised with planned case managers of dressing changes to review wound care regimes.
· I take urgent bloods and deliver within time restrictions ensuring results are available at the earliest opportunity.
