My role entails delivering healthcare interventions to clients 18 and above, in their home environment, this includes care homes.

Assumed a variety of nursing responsibilities

  • Conducting First visit assessments identifying health needs and concerns by implementing individualised care planes and holistic intervention. From initial assessment caring out risk assessments including environmental, patient and families/ friends' behaviours and calculating impacts to safety of patient and staff. Ensuring documented risk assessments, all involved aware and care plans such as double ups are implanted for safety and reviews are performed. Clinical risk assessments such as moving and handling, falls risk assessments and assessing patients' mobility and discussions of wants and needs regarding mobility. From assessment referring to correct services such as occupational therapy and falls services.
  • Safeguarding is an important aspect of my role ensuring clients are safe within their living environment, care and health needs are met. I undertake assessments to ensure patient has the capacity to make choices regarding care, health and living choices. If deemed unable to work within their best interest and liaising with services for advocates, potential refuge if required and implanting education and care services to keep patient at home if appropriate. Assessment with families/ carers to ensure understanding of patient health and care needs, providing education where appropriate, recognising signs of carers stress and implementing support to prevent crisis. I liaise with trust safeguarding team and Clinical leads when required, reporting all safeguarding's to local authorities, reporting all safeguarding via trusts digital incident and auditing system and liaising with Emergency services when required. Ensuring all documentation on patient record and updating all involved in care if appropriate. When required attending multidisciplinary meetings with acute hospital and other services to discuss plan of safety and care for patient.
  • Managed daily caseload, preformed daily allocations for team, triaging tasks, updating care outcomes from tasks from other services and collaboratively worked with interdisciplinary teams on or regarding visits.
  • End of life and Palliative support visits including providing emotional support to Families and patient during challenging times. Administering of pre-emptive medications and administering constant flow medication via syringe drivers and providing care for symptom management in the community preventing hospital admissions and working alongside patients' preferences. Discussions with patient and family on End of Life and care wishes. I liaise with specialist services to enhance support for patient and families at home, further education and treatment for symptom management and services for patient to attend support groups and bereavement support.

· Carried out infection control measures resulting in reduced risk of infections using aseptic technique. I also worked alongside infection control protocols for hospital acquired infection for Vancomycin-resistant enterococci and MRSA. Following all PPE requirements and prevention of cross infection from households.

  • Management of Diabetes - Blood glucose reading levels, observing ranges, acting on hypoglycaemic and hyperglycaemic episodes and calling emergency services if required. I administer insulin. Providing support and decision making for healthcare support workers on diabetic visits. I also educate and train patients in relation to diabetes management and self-administration. I liaise with GPs and Diabetic Specialist Nurses to review patients' management regimes if ineffective.
  • I administer anticoagulation medication, immunosuppressant medication and Vitamin injections. I educate and train patients in relation to the medication management and how to self-administer medication if appropriate.
  • I preform wound care which includes assessing the wounds, provide the correct dressing regimes, deliver education in relation to wound care and infection prevention pathways. For any wound infections I liaise with GPs re swabs and request medication where indicated. I am trained to carry out soft bandaging, compression, ABPI's and Dopplers to ensure compression is suitable on following a lower limb assessment and refer to specialist service on results of compression assessments if required. I am trained in negative pressure therapy enhancing wound healing. I preform wound reassessments including obtaining photographs to evidence deterioration or healing and if current regimes are effective, if ineffective working alongside Local wound pathways, clinical senior members of staff and specialist services. I deliver Food first education around the role of nutrition in healing and prevention as well as liaising with skin integrity teams including Tissue Viability and Lymphedema services. I assess any pressure areas and educate on repositioning, identify and order pressure relieving equipment, reporting incident through trusts digital, integrated risk, audit, and quality assurance system and referring to specialist per protocol. I deliver care to drain sites and monitor for infection or blockages and report patient to ESAC services if required.
