Part of my current role as a dedicated Diabetes Specialist Nurse, is being the named nurse for transition. Which means I predominantly work with young people aged 16-19 years living with diabetes, alongside their families. I focus on empowering adolescents to manage their condition effectively while navigating the unique challenges of this age group. I offer guidance on diabetes self-management, lifestyle modifications, and emotional well-being, ensuring that both the patient and their family feel supported throughout the process.

My current role includes both clinical, community-based and remote working and I always demonstrate exceptional time management and workload prioritization to ensure optimal service delivery. My strong organizational skills and attention to detail enable me to manage competing demands while maintaining a high level of efficiency and professionalism. My current role has also allowed me the opportunity to refine and expand my IT skills in a fast-paced, technology-driven environment.

A significant aspect of my current role involves delivering comprehensive diabetes education to patients and their families. I am skilled at providing high-quality, individualized education, tailoring my approach to meet the unique needs of each person. I ensure that the information is presented at a comfortable and manageable pace, fostering understanding and empowerment. My ability to assess whether education is best delivered in a group setting or through a personalized 1:1 session allows me to create an environment that promotes engagement and optimal learning outcomes.

I am highly skilled in the initiation and ongoing education of insulin pump therapy, including both stand-alone insulin pumps and hybrid-closed loop systems. With a deep understanding of these advanced technologies, I am committed to empowering individuals and healthcare teams with the knowledge and skills necessary to effectively use insulin pumps for optimal diabetes control. My expertise includes guiding patients through the initiation process, troubleshooting common challenges, and providing ongoing support to ensure success with insulin pump therapy. In line with recent TA guidance, which asserts that all children and young people with Type 1 Diabetes should have access to hybrid-closed loop systems, my team and I have been working diligently to ensure that every patient can make informed choices about the technology they use. In my current role, I have played a part in the development of a station rotation model for the delivery of group pump start initiation. This approach has significantly enhanced the effectiveness of education delivery while reducing lengthy waiting lists, ensuring that more patients can access hybrid-closed loop systems in a timely manner.

Our team's efforts were formally recognized with a nomination for a Trust Quality Improvement Award last year, underscoring the impact of our work on improving patient outcomes and service delivery. Additionally, our model has garnered attention from other diabetes teams, and it is now being adopted as a best practice across other services.
