A highly experienced palliative care nurse, currently working within the community Hospice at Home team which is commissioned to provide end of life care in the last 4 weeks of life, to patients who want to die at home, and also Hospice at home day and night sitting service including Marie Curie night sits

This involves managing and prioritising new referrals and then completing detailed patient care plans and assessments to ensure timely and cohesive coordination and delivery of personal care

Working closely with district nursing teams and other healthcare professionals involved in the patients care, helping to maintain good communication and clear documentation at all times

As shift lead duties include organising and planning the workload each day, handover to carers discussing any new patients, and discussing the current caseload identifying any new issues or problems that may need to be addressed, able to use discretion and confidentiality when required being proactive rather than reactive to any situations that arise

During the last 12 years I have worked across all areas of the hospice including our well-being center, outpatients/ pain clinic and our inpatient nursing unit
