I have 18 years experience in nursing (Accident and Emergency, Critical Care) with the last 6 spent within primary care in different settings. I am currently employed as an advanced nurse practitioner working for a digital GP practice in London. The practice has both an NHS and private arm and I work across the both seeing patients of all ages. I manage patient's with both acute and chronic conditions, and am responsible for the safe and effective remote assessment, diagnosis, treatment and review of patients presenting with a wide range of health needs. I also work on a locum basis within a couple of Out of Hours Services, seeing patient's of all ages with undifferentiated diagnoses.
I have found working with patients' who have undifferentiated diagnoses exciting and challenging. Formulating, managing and evaluating effective and timely treatment plans is always the focus along with the development of strong relationships with both patients and colleagues.
I strive to ensure that my practice is supported by knowledge of the relevant organisational policies and procedures and is informed by evidence based research and information. I am always keen to continue my professional development and keep updated through various means. I feel this is a cornerstone of safe and competent practice. Keeping up to date and knowledgeable is important in terms of maintaining safe boundaries, scope of practice and accountability for my autonomous practice.
I am aware of my limitations and strengths, and ensure that I am a safe practitioner and work within my scope of practice. I really value and welcome the support and mentorship from experienced practitioners and clinicians to enable me to become a more effective, versatile and knowledgeable member of the team who then finds pride in supporting others.
I learnt so much in my earlier career within the hospital, but have really enjoyed the challenges of primary care, I have been keen to remain in this field and independently sought out and self funded further education to support this role. I have completed a Masters in Advancing Professional Practice at Sheffield Hallam University. I successfully passed the Advanced Physical Assessment and Consultation Skills, Research for Practice, Paediatrics within the Acute and Primary Sector, Pain Management, Contemporary Issues within Advanced Practice (risk assessment, clinical decision making, leadership, and critical thinking) and Minor Illness modules, plus the dissertation. My Dissertation focussed on the factors which have an impact on using shared decision making within the effective prescribing of antibiotics. Outside of the MSc, I have completed the Independent Prescribing module (V300) in June 2019.
Having the opportunity to develop within my current role within a digital practice has been very interesting, I have found it to be innovative and progressive experience. I have taken part in an audit regarding how the service responds to a possible sepsis. My work is solely on a remote basis either by video or telephone consultation. I feel I have developed my IT, communication, and remote assessment skills in particular.
Along side this role, I have worked within an urgent care centre and an out of hours service. These roles together have been very complementary and I have valued these roles in particular as they have enabled me to continue to be practical and hands on. I have been lucky to have the opportunity to work remotely and hands on that are specific to that role. This opportunity has allowed me to maintain a good all round holistic approach to patient care.

My current role builds upon the skills/role developed in my previous urgent care practitioner out of hours role. I continue to undertake an autonomous role but have access to senior support if required from a GP
I see patients mainly on an urgent on the day basis with a range of undifferentiated and differentiated conditions. I see patients across all age groups.
As part of my role, I also manage some more long term conditions.
I manage workflow prescribing requests as part of a team and respond to urgent tasks as needed on the day. I undertake home visits as required.
I am competent with Docman and Emis. I am an independent prescriber.
I currently undertake both an autonomous role and team role in out of hours telephone triage, face to face consultations and home visits for a wide range of patients including infants, children, adults, mental health, palliative care and the elderly frail and complex patient. This involves sound clinical assessment, critical thinking and advanced decision making skills as well as liaising with a wide range of professional teams/healthcare professionals to co-ordinate the most appropriate, safe, effective and timely care for patients.
Dispense and administer medications according to patient group directives.
