Deliver advanced specialist care to patients with heart failure:

  • Undertake inpatient reviews, interpret echocardiography results, initiate and optimise treatment in line with NICE guidelines, and provide comprehensive patient education.
  • Arrange and coordinate inpatient transfers for advanced diagnostics (e.g. coronary angiography, cardiac MRI) and complex device therapies.
  • Identify inpatients suitable for early discharge and provide outreach support to the community to avert crisis and reduce avoidable hospital admissions.
  • Optimise heart failure pharmacological therapy to improve outcomes and avoid admission.
  • Prescribing and supporting delivery of IV furosemide and IV iron, within the day hospital.

  • Lead and manage the heart failure cardiac rehabilitation service on a daily basis, ensuring safe, effective, and efficient service delivery.
  • Manage the operation of the cardiac rehabilitation gym, including staffing and service organisation.
  • Conducting IPR and supporting personal development of staff.
  • Act as a specialist expert resource for staff across the Trust and community, providing advice and guidance on heart failure management.
