Current experience

• Band 5 Occupational Therapist with Community Recovery Service (CRS) at South Warwickshire NHS Foundation Trust.
• CRS delivers intermediate care, involving up to maximum of six weeks of free care, dependent on individual patient’s needs after stays in hospital. Carers and therapists work towards assessing and reducing care and increasing patients’ independence.
• Past seven months, focus has been on improving patient care and fostering recovery since qualifying. Dedication to this important to improve health and well-being of patients.
• In this role, have acquired extensive knowledge and honed skills in conducting various assessments.
• Effectively perform triaging assessments, excellent initial therapy assessments, occupational therapy assessments, skin assessments, and evaluations of appropriate care packages.
• Highly skilled in developing personalized treatment plans, implementing interventions, and planning for effective patient discharge.
• Strive to deliver high standard of care to patients with ranges of complex sensory and cognitive challenges within community settings.
• Independently and confidently lead Therapy Assistants to conduct and complete assessments through supervision and implementing training to wider therapy team.

Roles & Responsibilities

• To be professionally and legally accountable for designated case load of clients with variety of physical and social needs.
• To undertake comprehensive assessments of clients with wide variety of complex conditions using investigative and analytical skills of clinical and non-clinical information to form accurate diagnosis and prognosis.
• To formulate individualised therapy programmes, using sound clinical reasoning skills and evidence-based treatment to provide rehabilitation, which is client led and goal orientated.
• To work innovatively to address challenges posed by providing rehabilitation in variety of community settings.
• To have clinical responsibility for designated caseload and at times dealing with unplanned and unpredictable urgent referrals for clients living in community based settings.
• To demonstrate ongoing evaluation of therapeutic.
