  • To carry out a clinical review relating to the death of a person in the custody (or released on temporary licence) of a prison, Young Offender Institution, Secure Children's Home, Immigration Removal Centre, Courts or Approved Premises.
  • To undertake level 1, 2 and 3 investigations into the clinical circumstance surrounding the death, working closely and making early contact with Prison and Probation Ombudsman (PPO) Investigator.
  • To produce accurate and timely reports following the NHS England guidance.
  • To provide a comprehensive review of care including an evaluation of equivalence of care in secure environments with care in the community.
  • To raise any issues of concern that requires immediate attention to Commissioners,via the Death in Custody Service Manager.
