Working across three GP surgeries supporting service users who have a diagnosis of a serious mental illness or mental health difficulties with provision of advice, education, support and interventions to patients and their carers. Another aspect is identifying support to carers and offering careers assessment and review of patient care plans against

Work with older adults who have a diagnosis of dementia/mental health concerns. I review my own caseload of 79 patients every 3-6 months providing advice where appropriate offering to help to both to the person and their family in dealing with memory difficulties with appropriate treatment to the diagnosis increasing confidence. Providing a follow-up service to offer further help if/when their needs change;

Working across the Learning disability team, MCA and domestic abuse team at St Georges hospital. I was responsible for ensuring that the safety and wellbeing of adults is protected. Responsibilities include but not subjected to being first point of contact for all triage, actioning referrals onwards to social services and providing updates to MDT and by reporting concerns of abuse to statutory authorities

Home Instead Care is the UK’s leading independent domiciliary care company,providing quality, person centered services to self-funding clients in their own home. My role was to assess the needs of service users and plan and evaluate care using a collaborative
approach with service users and their carers within the Wandsworth/Lambeth and Dulwich
care network who had dementia/mental health problems working alongside multi-disciplinary
team to ensure effective care delivery.
Gained my NVQ level 3 in health and social care while alongside my day-to-day role. I was involved in
helping to obtain an outstanding CQC rating in 2020 with my assistance of field assessments
and QA/service review and keeping up to date care plan.

Made redundant due to pandemic

Responsible for over 40% of the Women’s fashion division by developing and deliver an extensive range
within Jersey, knitwear, and sleepwear increased profit margin on nightwear and this expanded the division and enhanced department margin by 5%.
