I have worked for clients group with multiple complex needs, and the diagnosis ranges from Head Injury, Stroke, Learning Disabilities, ADHD, Autism, Down Syndrome, Epilepsy, Developmental Disorders, Fracture and Dementia. The majority of my work was based in the client's home in the community or supported living settings and also, supporting the client while staying in hospital for ongoing rehabilitation programs. My job responsibilities include:
Supporting clients via verbal and nonverbal communication methods by following care plans to ensure the health, safety and well-being of the clients. It includes arranging interactive discussion sessions, organizing social events, assisting in hospital visits and therapy sessions, and using audio-visual aids like tablet computers for learning and entertainment.
Assisting Manual Handling activities by using different types of equipment such as ceiling track hoist, mobile hoist, stand-aids, compatible slings to lift and transfer clients to wheelchairs, toilet and bath chairs etc. Also, assisting clients to walk with mobility aids i.e. Zimmer frames.
Assisting in 24-hour Postural Management that involves changing positions and postures throughout the day by using appropriate transfer techniques and manual handling equipment such as multi-functional beds, riser recliner chairs, sliding sheets etc. Also, supporting other MDTs (OT, PT & SLT) to assess and recommend manual handling equipment and supporting aids for service users to meet their care needs.
Training, assisting and promoting Activities of Daily Living, including feeding, toileting, bathing, dressing, grooming, washing and cleaning. While the focus remains on achieving the maximum optimum functionality of the clients by using appropriate equipment and techniques.
Food administrations including cooking, feeding and PEG feeding (fortified food formulas via PEG device and tubes).
Administering medications, including mouth-fed and PEG-fed medicines, PRNs and emergency medicines such as Buccolam Midazolam etc. Also, maintaining MAR charts, ordering medicine in due time, and booking appointments via phone calls, emails and apps.
Keeping records of activities and documentation of incidents by maintaining paper-based and electronic logbooks such as communication notes, incident books, hospital and medical notes, temperature and weight observations etc. Then, regularly discuss them with the supervisor and other health care professionals for planning activities and evaluate the progress of the rehabilitation plans.
Following strength-based approaches to promote opportunities for service users and their family members to empower and be actively involved in the decision-making process for their care input. Also, helping the clients in day-to-day decision-making by considering their mental capacity and taking person-centred approaches.
Ensuring the physical, psychological and emotional well-being of the client and related parties by safeguarding vulnerable people and whistleblowing in case of negligence or abuse, following the local safeguarding policies.
