Summary

I have 9 years experience of working with vulnerable families both children and adults. I have always had a passion for helping others and making a positive change. I am a hard working, proactive individual who likes to be challenged and not afraid to ask for support if i am not sure of something. I am very motivated and like to reach the end goal on whether it is helping an individual or achieving something for myself.

I have excellent time keeping skills and good communications with both professionals and clients. By having good communication it helps to build a trustworthy relationship.

I have good IT skills and know how to use different programs and databases from the sixth local authorities in Berkshire.

- Taking emergency referrals for the six local authorities in Berkshire.

-The referrals will then be logged on to Liberty create where the referral will then get allocated out to a social worker or mental health practitioner.

-Completing database checks using mosaic, Paris, ICS, IAS, West Berkshire portal, connected care and LAS.

-Signposting callers to other services where appropriate.

-Liaising with social workers and mental health professionals about the referrals that are coming in to the service

- Good communication skills with all colleagues.

-Acting as an appropriate adult when needed for vulnerable adults or children who are in custody.

-Ensuring there is good communication on the phone to the referrers, the referrals come from a variety of people such as professionals or individuals themselves. At times the individuals calling it can be at crisis point where they needed support immediately. On one several occasions i have had to call the police for a welfare check on an individual due to the concern for there mental health. This was done whilst keeping the individual on the phone talking.

  • I was responsible for supporting 13 individual families, both adults and children.
  • I completed assessments with both the adults and the children identify what support was needed and what support they had in place.
