Summary

I currently work at HMP Preston. The health care is provided by a private company named Spectrum Community Health CIC and this is the lead healthcare organisation for local prisons in the Northwest.

I am employed as an Assistant Practitioner within the prison. When I was first employed at HMP Preston I came as a social care assistant which I did for 12 months then I had the opportunity to progress to a healthcare support worker, after a further 12 months I got a great progression opportunity to complete the Assistant practitioner degree, this leads me to my job role today.
I applied for this role because I am passionate about caring for others and interested in improving patients’ health and wellbeing. My current experience and skills have enabled me to gain the confidence to progress in my career.
As part of my role, I am responsible for ensuring that the appropriate interventions are carried out to meet patient’s physical health needs which include completing personal hygiene needs, assisting with diet and fluid intake. I always use the, Assess, Plan, Implement and Evaluate tool to ensure that I have followed the process through to the end.
Communication is an essential element of nursing when delivering care to patients, I have a wide range of communication and inter-personal communication skills that I use to be able to relate to patients when delivering person-centred care. Ensuring that documentation is contemporaneous throughout.
I am an advocate for my patients and their safety is my priority, so protection is also an integral part of my current role. I support patients with dignity and respect, in addition I actively promote their independence when needed. Providing safe, personal, and effective care is at the heart of everything I do to ensure that all care is patient centred.
I also have a responsibility for ensuring that I accurately document the care and interventions that I have given in the patient’s individual care plans on the online data base, reporting any concerns to the nurse in charge.
I can work independently as an automatous practitioner or as part of a team whilst responding effectively to instructions from senior nursing staff.

Responsible and patient dedicated to improving patient health and well-being using comprehensive training and education. Offers strong interpersonal and communication skills to quickly build trust and maintain patient comfort. Keeps strong working knowledge of NHS policies and procedures.
