A family group conference is an early intervention service and is used whenever a situation is sufficiently serious that a plan and decision needs to be made about a vulnerable child/child.
I undertook the role of early help case management, completing assessments and to support families in assessing short break services, providing advice and guidance in relation to local support as well as signposting and referring to other agencies in order to deliver the best outcome for each service user.
This role was based in children's social care, children with disabilities team:
I supported children/ young people in a residential setting with autistic spectrum disorders, learning disabilities and other associated complex needs
I provided complex medial, physical and learning disability specialist nursing care and end of life care for children, all of which was provided within a homecare setting.
